结直肠癌从痈疽辨治的理论依据与临床运用

摘　　要：结直肠癌是常见的消化系统恶性肿瘤,其发病率与死亡率呈现逐年上升的态势。结合历代古籍与临床实践,结直肠癌的发生发展与痈疽类疾病在病因病机、病程转归、病症特点等方面存在共性,故而从痈疽理论立法,对开展结直肠癌的分期辨治具有临床指导意义。在治疗方面,中医内治以消、托、补为核心,病程前期以消法为主,散结清热、抗癌消痈;中期以透托为贵,透邪解毒、防其传变;后期以补托为要,补中寓攻、顾护正气;若癌毒将尽,机体历经攻伐而气血大伤,则以补法为重。此外,也应当注重将中医外治与现代医学疗法有机结合,适时介入中医外治手段。Colorectal cancer is a common malignant tumor of digestive system,and its morbidity and mortality are increasing year by year.Based on the review of ancient medical books and the clinical practice,it is found that during the occurrence and development of disease,colorectal cancer and the diseases of carbuncle and gangrene share common things in the etiology and pathogenesis,course of disease and prognosis,and clinical manifestation features.Therefore,it is feasible to make staging differentiation and treatment of colorectal cancer following the theory of carbuncle and gangrene.The internal treatment of traditional Chinese medicine for colorectal cancer can be achieved by the methods of resolution,expulsion,and tonification.At the early stage of colorectal cancer,the resolution method is stressed for dissipating mass and clearing heat,and inhibiting tumor and dissolving abscesses;at the middle stage of colorectal cancer,the expulsion method is stressed for expelling pathogens and removing toxins,and preventing the transmission and change of the disease;at the later stage of colorectal cancer,the expulsion-tonification method is stressed for tonifying during expelling pathogens,and preventing the healthy Qi.If the cancer toxin is almost removed out and the body is seriously injured in terms of Qi and blood after elimination therapy,the tonifying method should be used firstly.Moreover,attention should be paid to the timely intervention with combined use of external treatment of traditional Chinese medicine and modern medical therapy.The discussion on the treatment of colorectal cancer following the theory of carbuncle and gangrene will provide a new idea for clinical treatment of colorectal cancer.
