摘　　要：淋巴漏是妇科恶性肿瘤腹腔镜术后一种较少见的并发症,其症状在中医学中当属“水饮病”“水气病”范畴,其病机复杂,目前尚无标准的治疗方案。淋巴结为过滤淋巴液的通道,与中医“三焦为水液通行之道”不谋而合,恶性肿瘤手术涉及淋巴结清扫,在一定程度上干涉了水液的正常循行。王小红教授认为术后淋巴漏的根本病机为“正气亏虚,三焦气化失调,水液不循常道”,结合多年临床经验,承古今医家之说,王小红教授注重“异病同治”,治疗主方选用苓桂术甘汤,以改善症状、温阳化饮、恢复三焦气化为主要目标,同时兼证用药加减。Lymphatic leakage is a rare complication after laparoscopic surgery for gynecological malignant tumors.Its symptoms can be classified into the categories of“fluid retention”and“retained fluid”in the field of traditional Chinese medicine.Its pathogenesis is complex,and there is no standard treatment regimen for lymphatic leakage till now.Lymph nodes are the channels for filtering lymph fluid,which accords with the function of the triple-energizer,the pathway of water and liquid in the traditional Chinese medicine.Lymph node dissection was performed during the malignant tumor operation,which interferes with the normal flow of water and liquid to a certain extent.Professor Wang Xiao-hong believes that the fundamental pathogenesis of postoperative lymph leakage is due to healthy qi deficiency,triple-energizer qi transformation disorder,water and liquid failing in circulating in the normal path.Based on years of clinical experience and following the thoughts of ancient and modern Chinese medical practitioners,Professor Wang Xiao-hong stresses the principle of“the same treatment for different diseases”,and usually adopts the classic prescription Linggui Zhugan Decoction to relieve the symptoms,warm yang to resolve fluid-retention,and restore the function of qi transformation of triple-energizer.Moreover,modification of the herbs for the complicated syndrome following the principle of“medication of the herbs for the exact syndrome”.
