应用球囊逼出试验诊断排便障碍型便秘的临床价值研究

摘　　要：目的探讨球囊逼出试验诊断排便障碍型便秘的临床价值。方法随机选取2020年7月至2022年7月期间浙江省永嘉县人民医院收治的60例功能性便秘患者,对所有纳入患者均行球囊逼出试验和肛门直肠压力测定检查,且以肛门直肠压力测定结果为金标准,并分析比较分析排便障碍型便秘患者症状特点及两种检查方式在诊断中的一致性。结果排便障碍型便秘患者中出现排便费力25例、粪便干结18例、肛门堵塞感8例、排便不尽14例及肛门坠胀感12例,非排便障碍型便秘患者中出现排便费力17例、粪便干结8例、肛门堵塞感2例、排便不尽6例,无患者表现出肛门坠胀感,两组间临床症状差异有统计学意义(P<0.05);60例功能性便秘患者中经肛门直肠压力测定为排便障碍型便秘35例,其中球囊逼出试验阳性25例,在非排便障碍型便秘组中阴性18例,球囊逼出试验与肛门直肠压力测定检查在诊断排便障碍型便秘患者中的一致性较高(71.43%),且球囊逼出试验在排便障碍型便秘的诊断中具有良好的效能(P<0.001);球囊逼出试验操作时间(7.54±0.87)min显著短于肛门直肠压力测定(21.38±1.14)min,差异有统计学意义(P<0.001);球囊逼出试验患者满意度93.33%,高于肛门直肠压力测定(90%),两种检查方式满意度无显著差异(P>0.05)。结论球囊逼出试验在排便障碍型便秘患者的诊断中具有良好的诊断效能,价格便宜、操作时间短且患者满意度较高,未来可在基层医院推广普及。Objective To explore the clinical value of balloon ejection test in the diagnosis of constipation with defecation disorders.Methods 60 patients with functional constipation admitted to our department from July 2020 to July 2022 were randomly selected,and all included patients underwent balloon extraction test and anorectal pressure measurement.The results of anorectal pressure measurement were used as the gold standard,and the symptom characteristics of patients with defecation obstruction constipation and the consistency of the two examination methods in diagnosis were analyzed and compared.Results Among patients with defecation disorder constipation,there were 25 cases of laborious defecation,18 cases of dry stool,8 cases of blockage,14 cases of incomplete defecation,and 12 cases of anal bloating.Among patients with non-defecation disorder constipation,there were 17 cases of laborious defecation,8 cases of dry stool,2 cases of blockage,and 6 cases of incomplete defecation.No patients showed anal bloating,and there was a statistically significant difference in clinical symptoms between the two groups(P<0.05).Among the 60 patients with functional constipation,35 were diagnosed as dysfunctional constipation through anorectal pressure measurement.Among them,25 were positive for the balloon push out test and 18 were negative in the non-dysfunctional constipation group.The consistency between the balloon push out test and anorectal pressure measurement in the diagnosis of dysfunctional constipation was high(71.43%),and the balloon push out test had good efficacy in the diagnosis of dysfunctional constipation(P<0.05).The operation time of the balloon extraction test(7.54±0.87 minutes)was significantly shorter than that of the anorectal pressure measurement(21.38±1.14 minutes),and the difference was statistically significant(P<0.05).The satisfaction rate of patients with balloon extraction test was 93.33%,which was higher than that of anorectal pressure measurement(90%).There was no significant difference in satisfac
