摘  要:目的 探讨同源盒基因A11(HOXA11)和CD10在子宫内膜异位症相关卵巢癌(EAOC)中的表达及其临床意义。方法 回顾性选取2021年5月至2022年6月入同济大学附属第一妇婴保健院接受手术治疗患者的EAOC组织标本96例作为研究对象,同时收集单纯子宫内膜异位症患者的子宫内膜组织50例作为对照。采用免疫组织化学染色法检测EAOC组织和子宫内膜异位症组织中HOXA11和CD10蛋白的表达,并比较子宫内膜异位症和EAOC组织标本中蛋白表达阳性率,分析HOXA11和CD10蛋白与临床病理特征的关系;Spearman相关性分析HOXA11与CD10的相关性;采用受试者工作特征曲线分析HOXA11和CD10对EAOC的诊断效能。结果 子宫内膜异位症中HOXA11和CD10蛋白的表达较少,EAOC中,HOXA11和CD10蛋白分布较多,主要表达在细胞膜中,呈现棕黄色或者淡黄色。EAOC组织中HOXA11和CD10蛋白表达阳性率分别为76.04%、64.58%,明显高于子宫内膜异位症组织(14.00%、30.00%),差异均有统计学意义(P<0.05)。Ⅲ~Ⅳ期、低分化、有淋巴结转移患者的HOXA11表达阳性率分别为88.52%、87.10%、88.37%,均明显高于Ⅰ~Ⅱ期、高中分化、无淋巴结转移患者(54.29%、70.77%、66.04%),CD10蛋白表达阳性率分别为77.05%、80.65%、76.74%,均明显高于Ⅰ~Ⅱ期、高中分化、无淋巴结转移患者(42.86%、56.92%、54.72%),差异均有统计学意义(P<0.05)。HOXA11与CD10的表达呈正相关,差异有统计学意义(r=0.573,P=0.031)。当截断值为3.62时,HOXA11诊断EAOC的曲线面积(AUC)为0.826(95%CI:0.768~0.883),敏感度为83.16%,特异度为54.74%;当截断值为2.97时,CD10诊断EAOC的AUC为0.767(95%CI:0.699~0.835),敏感度为71.58%,特异度为58.95%;HOXA11+CD10联合检测诊断EAOC的AUC为0.871(95%CI:0.817~0.924),敏感度为86.32%,特异度为67.37%。结论 HOXA11和CD10蛋白在EAOC组织表达升高,与临床分期、分化程度、淋巴结转移密切相关,且HOXA11和CD10联合诊断具有一定的诊断效能�Objective To investigate the expression and clinical significance of homeobox genes A11(HOXA11)and CD10 in endometriosis associated ovarian cancer(EAOC).Methods Ninety-six EAOC tissue samples from patients admitted to Shanghai First Maternity and Infant Hospital,School of Medicine,Tongji University from May 2021 to June 2022 for surgical treatment were retrospectively selected as the study objects,and 50 endometriosis tissue samples from patients with simple endometriosis were collected as the control.The expression of HOXA11 and CD10 protein in EAOC tissues and endometriosis tissues was detected by immunohistochemical staining,the positive rate of protein expression in Endometriosis and EAOC tissue samples was compared,and the relationship between HOXA11 and CD10 protein and clinicopathological characteristics was analyzed.The correlation between HOXA11 and CD10 was analyzed by Spearman correlation,and the diagnostic efficacy of HOXA11 and CD10 in EAOC was analyzed by receiver operating characteristic curve.Results The expression of HOXA11 and CD10 protein in endometriosis was less than that in EAOC,HOXA11 and CD10 protein were more distributed,mainly expressed in the cell membrane,showing brown or light yellow.The positive rates of HOXA11 and CD10 protein expression in EAOC tissue were 76.04%and 64.58%,respectively,which were significantly higher than those in endometriosis tissue(14.00%and 30.00%),and the differences were statistically significant(P<0.05).The positive rates of HOXA11 expression in patients with stageⅢ-Ⅳ,low differentiation,and lymph node metastasis were 88.52%,87.10%,and 88.37%,respectively,which were significantly higher than those in patients with stageⅠ-Ⅱ,high differentiation,and no lymph node metastasis(54.29%,70.77%,and 66.04%),the positive rates of CD10 protein expression were 77.05%,80.65%,and 76.74%,respectively,which were significantly higher than those in patients with stageⅠ-Ⅱ,high differentiation,and no lymph node metastasis(42.86%,56.92%,and 54.72%),the differences wer
