食管动力指标联合食管pH值检测在胃食管反流病合并抑郁患者病情评估中的临床价值

摘　　要：目的 研究食管动力指标联合食管pH值检测在胃食管反流病(GERD)合并抑郁患者病情评估中的临床价值。方法 以回顾性分析为法,观察对象为2020年1月至2022年1月入海南医学院第一附属医院的80例GERD患者,参考是否合并抑郁分为研究组(合并抑郁,37例)与对照组(无抑郁,43例)。比较两组患者食管动力指标、食管pH值;同时,比较不同类型GERD[反流性食管炎(RE),非糜烂性反流病(NERD)]合并抑郁患者的食管动力指标、食管pH值及抑郁自评量表(SDS)评分,并分析SDS评分与食管动力指标、食管pH值的相关性。结果 研究组远端收缩积分(DCI)、食管下括约肌压力(LESP)均明显低于对照组,DeMeester评分、pH<4大于5 min反流次数、pH<4反流次数、立位pH<4的时间比率、pH<4的总时间比率、收缩前沿速度(CFV)均明显高于对照组,差异均有统计学意义(P<0.05)。研究组GERD合并抑郁患者中,RE患者的LESP明显低于NERD患者,SDS评分、DeMeester评分、pH<4大于5 min反流次数、pH<4的总时间比率明显高于NERD患者,差异均有统计学意义(P<0.05)。SDS评分与DeMeester评分、pH<4反流次数、pH<4的总时间比率呈正相关(P<0.05),与LESP呈负相关(P<0.05)。结论 GERD合并抑郁患者食管动力普遍呈现减弱状态,抑郁状态与食管动力、食管pH值呈现一定相关性,通过对患者进行食管动力指标联合食管pH值检测可有效评价患者病情状态,对评估患者病情及疗效有一定价值。Objective To study the clinical value of esophageal motility index combined with esophageal pH value in the evaluation of patients with gastroesophageal reflux disease(GERD)complicated with depression.Methods With the method of retrospective analysis,80 patients with GERD admitted to the First Affiliated Hospital of Hainan Medical College from January 2020 to January 2022 were observed.They were divided into study group(37 patients with depression)and control group(43 patients without depression)according to whether they had depression or not.The esophageal motility index and esophageal pH value were compared between the two groups.At the same time,the esophageal motility index,esophageal pH value and self-rating depression scale(SDS)scores of different types of GERD[reflux esophagitis(RE),non-erosive reflux disease(NERD)]with depression were compared,and the correlation between SDS score and esophageal motility index and esophageal pH value was analyzed.Results The distal contraction score(DCI)and lower esophageal sphincter pressure(LESP)in the study group were significantly lower than those in the control group,and the DeMeester score,>5 min reflux times with pH<4,reflux times with pH<4,the time ratio of standing position with pH<4,the total time ratio with pH<4 and the contraction front velocity(CFV)were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Among GERD patients with depression in the study group,the LESP of RE patients was significantly lower than that of NERD patients,and SDS score,DeMeester score,>5 min reflux times with pH<4 and the total time ratio with pH<4 were significantly higher than those of NERD patients,the differences were statistically significant(P<0.05).SDS score was positively correlated with DeMeester score,reflux times with pH<4 and total time ratio with pH<4(P<0.05),but negatively correlated with LESP(P<0.05).Conclusion The esophageal motility of GERD patients with depression is generally weakened,and the depressed state
