Vitapex糊剂根管充填治疗牙髓牙周联合病变患者的效果

摘　　要：目的:观察Vitapex糊剂根管充填治疗牙髓牙周联合病变患者的效果。方法:选取2019年8月至2020年2月该院收治的125例牙髓牙周联合病变患者进行前瞻性研究,按照抽签法将其分为对照组(62例)和观察组(63例)。两组均行根管充填治疗,对照组予以氧化锌丁香油糊剂充填,观察组予以Vitapex糊剂充填,两组均治疗1个月,并随访2年。比较两组临床疗效、治疗前后炎性因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、IL-1β]水平、牙周相关指标[探诊深度(PD)、临床附着水平(CAL)、根尖周指数(PAI)]水平及根管治疗约诊间痛(EIAP)发生率。结果:观察组治疗总有效率为95.24%(60/63),高于对照组的79.03%(49/62),差异有统计学意义(P<0.05);观察组IL-6、TNF-α、IL-1β水平均低于对照组,差异有统计学意义(P<0.05);观察组PD、CAL、PAI水平均低于对照组,差异有统计学意义(P<0.05);观察组EIAP发生率为14.29%(9/63),低于对照组的30.65%(19/62),差异有统计学意义(P<0.05)。结论:Vitapex糊剂根管充填治疗牙髓牙周联合病变患者可提高治疗总有效率,降低炎性因子水平和EIAP发生率,改善牙周相关指标水平,效果优于氧化锌丁香油糊剂充填治疗。Objective:To observe effects of Vitapex paste root canal filling in treatment of patients with periodontic endodontic lesions.Methods:A prospective study was conducted on 125 patients with periodontic endodontic lesions admitted to the hospital from August 2019 to February 2020.According to the lottery method,they were divided into control group(62 cases)and observation group(63 cases).Both groups were treated with root canal filling.The control group was filled with Zinc oxide eugenol paste,while the observation group was filled with Vitapex paste.Both groups were treated for 1 month and followed up for 2 years.The clinical efficacy,the levels of inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),IL-1β],the periodontal related index levels[probing depth(PD),clinical attachment level(CAL),periapical index(PAI)],and the incidence of inter-appointment pain(EIAP)in root canal therapy were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the observation group was 95.24%(60/63),which was higher than 79.03%(49/62)in the control group,and the difference was statistically significant(P<0.05).The levels of IL-6,TNF-α and IL-1β in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The levels of PD,CAL and PAI in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of EIAP in the observation group was 14.29%(9/63),which was lower than 30.65%(19/62)in the control group,and the difference was statistically significant(P<0.05)Conclusions:Vitapex paste root canal filling can improve the total effective rate of treatment,reduce the levels of inflammatory factors and the incidence of EIAP in the patients with periodontic endodontic lesions,improve the periodontal related index levels.Moreover,it is superior to Zinc oxide clove oil paste filling treatment.
