摘  要:目的通过免疫组化双染法在甲状腺乳头状癌(papillary thyroid carcinoma,PTC)组织中的应用,建立人工判读标准并探索该标准应用的可行性;分析非桥本背景PTC组织中T、B淋巴细胞(简称T细胞、B细胞)的浸润特征与临床病理特征的相关性。方法A组选择16例PTC组织进行免疫组化双染和多重免疫荧光染色,分别进行人工判读和机器软件判读;通过Kappa值对比分析人工判读和机器判读结果的一致性。B组选择27例非桥本背景PTC组织进行免疫组化双染,采用人工判读肿瘤组织中T、B细胞表达水平。Kaplan-Meier曲线和Log-rank检验进行无瘤生存期(disease free survival,DFS)分析。结果免疫组化双染人工判读与多重免疫荧光染色机器判读结果达到中等一致性(T细胞Kappa=0.508,B细胞Kappa=0.538),与免疫组化双染机器判读结果达到高度及中等一致性(T细胞Kappa=0.754,B细胞Kappa=0.418)。T细胞表达与各项临床病理特征均无相关性(P>0.05)。B细胞表达仅与淋巴结转移癌被膜侵犯、N分期有相关性(P<0.05)。生存分析显示,B细胞表达与患者DFS无相关性(P>0.05)。结论免疫组化双染人工判读标准用于肿瘤免疫细胞浸润的研究具有一定可行性。非桥本背景PTC中B细胞表达高低与淋巴结转移及N分期有关。Purpose To apply the immunohistochemical double-staining(IHC-D-S)method in papillary thyroid carcinoma(PTC)tumor tissues to establish manual interpretation criteria,and to analyze the correlation of T and B lymphocyte(referred to as T cell and B cell)infiltration characteristics with clinicopathological features in PTC tumor tissues without Hashimoto background.Methods In group A,16 tumor tissues of PTC patients were selected for IHC-D-S and multiple immunofluorescence staining(M-IF-S).Manual interpretation of IHC-D-S results and machine software interpretation of IHC-D-S and M-IF-S results were performed.Consistency of T and B cell expression interpretation results in tumors by Kappa value comparison was studied between manual and machine interpretation methods.In group B,27 tumor tissues of patients without Hashimoto background PTC were selected for IHC-D-S,and the expression levels of T and B cells in tumor tissues were read by manual reading criteria.Kappa values were applied to compare the degree of agreement between manual interpretation and machine interpretation for the analysis of high and low expression of T and B cells within the tumor.Kaplan-Meier curves and Log-rank tests were used for disease-free survival(DFS)analysis.Results The manual interpretation of IHC-D-S was in moderate agreement with the machine interpretation of M-IF-S(T-cell Kappa=0.508,B-cell Kappa=0.538)and in high and moderate agreement with the machine interpretation of IHC-D-S(T-cell Kappa=0.754,B-cell Kappa=0.418).The expression of T lymphocytes was not statistically different from all clinicopathological features(P>0.05).The expression of B lymphocytes was statistically different from lymph node invasion and N-stage(P<0.05).B-cell expression was divided into high and low expression groups for disease-free survival(DFS)analysis,which was not statistically significant(P>0.05).Conclusions The manual interpretation of IHC-D-S criteria is feasible for the study of tumor immune cell infiltration.The level of intra-tumor B lymphocyte exp
