胃腺癌中G6PC和Ki-67蛋白表达的临床病理学意义

摘　　要：目的探讨胃腺癌组织中G6PC和Ki-67蛋白表达及临床病理学意义。方法采用免疫组化EnVision法检测129例胃腺癌和126例正常胃黏膜组织中G6PC和Ki-67蛋白表达,运用生物信息数据库分析其与胃腺癌临床病理特征及预后的关系。结果胃腺癌组织中G6PC和Ki-67蛋白高表达率分别为62.0%、65.9%,明显高于癌旁正常胃黏膜组织(16.7%、30.2%)(P均<0.05)。Spearman相关分析结果显示,胃腺癌组织中G6PC与Ki-67表达呈正相关(P<0.05)。G6PC蛋白高表达与胃腺癌分化程度(χ^(2)=6.808,P=0.009)、脉管浸润(χ^(2)=4.525,P=0.033)及淋巴结转移(χ^(2)=8.371,P=0.004)相关。Kaplan-Meier法分析显示,G6PC蛋白高表达胃腺癌患者的总生存期明显低于G6PC蛋白低表达患者(P<0.001),与数据库分析结果趋向一致。Cox单因素和多因素分析显示,G6PC高表达不能作为胃腺癌患者死亡风险的独立因素。结论G6PC蛋白在胃腺癌组织中显著高表达,且与Ki-67表达呈正相关,可作为胃腺癌患者预后不良的潜在生物学标志物。Purpose To explore the expression of G6PC and Ki-67 proteins in gastric adenocarcinoma(GAC),as well as its clinicopathological significance in GAC.Methods The EnVision method of immunohistochemistry was applied to detect the expression level of G6PC and Ki-67 in 129 GACs and 126 normal gastric mucosa tissues,and bioinformatic analysis was combined to observe correlation between G6PC and clinicopathological characteristics as well as prognosis of GAC.Results Results of immunohistochemistry showed that G6PC along with Ki-67 were highly expressed in 62.0%and 65.9%of GAC tissues respectively,which was significantly higher than that in normal gastric mucosa tissues(16.7%and 30.2%,respectively)(P all<0.05).Spearman correlation analysis indicated that G6PC was positively correlated with Ki-67 in GAC(P<0.05),and high expression of G6PC was significantly correlated with tumor differentiation(χ^(2)=6.808,P=0.009),vessel invasion(χ^(2)=4.525,P=0.033)and lymph node metastasis(χ^(2)=8.371,P=0.004)in GAC.Besides,Kaplan-Meier analysis showed that GAC patients with high expression of G6PC had lower overall survival compared to those with low expression of G6PC(P<0.001),which was consistent with the result from database analysis.However,Cox univariate and multivariate analysis showed that G6PC was not an independent risk factor for GAC patients yet.Conclusion G6PC is highly expressed in GAC and positively correlated with Ki-67,which may be the potential marker of poor prognosis in GAC.
