新辅助化疗联合胸腹腔镜McKeown食管癌根治术治疗食管中下段癌的效果观察

摘　　要：目的观察新辅助化疗联合胸腹腔镜McKeown食管癌根治术治疗食管中下段癌的效果。方法104例食管中下段癌患者随机分为两组,手术组应用胸腹腔镜McKeown食管癌根治术治疗,联合组在对照组基础上应用术前新辅助化疗,比较两组的临床疗效、血清肿瘤标志物水平和并发症。结果联合组2年、3年的局部复发率、远处转移率均低于手术组,治疗后的CA125、CYFRA21-1、CEA水平均低于手术组(P<0.05)。结论新辅助化疗联合胸腹腔镜McKeoWn食管癌根治术治疗食管中下段癌患者效果显著,可有效降低血清肿瘤标志物水平。Objective To observe the effect of neoadjuvant chemotherapy combined with thoracoscopic and laparoscopic McKeown radical resection of esophageal cancer in the treatment of middle and lower esophageal cancer.Methods 104 patients with middle and lower esophageal cancer were randomly divided into two groups.The surgery group was treated with thoracoscopic and laparoscopic McKeown radical resection of esophageal cancer,and the combined group was treated with preoperative neoadjuvant chemotherapy on the basis of the control group.The clinical efficacy,serum tumor marker levels and complications were compared between the two groups.Results The 2-year and 3-year local recurrence rates and distant metastasis rates of the combined group were lower than those of the surgery group,and the levels ofCA125,CYFRA21-1 and CEA after treatment were lower than those of the surgery group(P<0.05).Conclusions Neoadjuvant chemotherapy combined with thoracoscopic and laparoscopic McKeown radical resection of esophageal cancer in the treatment of patients with middle and lower esophageal cancer has significant effect,which can effectively reduce the levels of serum tumor markers.
