摘　　要：目的 探讨^(18)F-PSMA-1007 PET/CT显像联合盆腔MRI对前列腺癌(PCa)患者术前评估的价值。方法 回顾性分析2019年5月至2022年1月本院^(18)F-PSMA-1007 PET/CT显像及盆腔MRI检查的46例可疑PCa患者资料。采用SPSS 22.0及MedCalc软件进行相关数据分析。结果^(18)F-PSMA-1007 PET/CT对PCa诊断的灵敏度、特异度、阳性预测值、阴性预测值及准确率分别为100%、40.0%、77.5%、100%、80.4%,二者联合可将特异度、阳性预测值及准确率分别提高至80.0%、91.2%、93.5%,特异度及阳性预测值的差异有统计学意义(P<0.05)。三种参数[最大标准化摄取值(SUV_(max))、ADC、胆碱(Cho)+肌酸(Cre)枸橼酸盐/(Cit)比值]诊断PCa的曲线下面积(AUC)分别为0.963、0.895、0.928。另外,^(18)F-PSMA-1007 PET/CT发现骨盆外淋巴结转移5例,骨转移8例,肺转移2例。结论^(18)F-PSMA-1007 PET/CT显像联合盆腔MRI可以提高PCa诊断准确率,尤其是前者可发现多部位转移灶,有助于PCa患者术前准确评估,指导临床医师制订合理的个体化诊疗方案。Objective To explore the value of ^(18)SF-PSMA-1007 PET/CT imaging combined with pelvic MRI in the pre-operative evaluation of patients with prostate cancer.Methods Forty-six patients who were suspected with prostate cancer underwent ^(18)SF-PSMA-1007 PET/CT imaging and pelvic MRI examination in our hospital from May 2019 to January 2022 were retrospectively reviewed.SPSS 22.O and MedCalc software were used for data analysis.Results The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of ^(18)SF-PSMA-1007 PET/CT were 100%、40%77.5%、100%and 80.4%,respectively.Combined imaging of ^(18)8F-PSMA-1007 PET/CT and MRI can help to improve its efficiency,with specificity of 80%,positive predictive value of 91.2%andaccuracy of 93.5%,and there were statistically significant differences in specificity and positive predictive value(P<0.05).The AUC values of the three parameters(SUV_(max),ADC,Cho+Cre/Cit)in the diagnosis of prostate cancer were 0.963,0.895,and 0.928,respectively.In addition,^(18)F-PSMA-1007 PET/CT detected 5 cases of lymph node metastasis and 8 cases of bone metastasis outside the pelvis,and 2 cases of lung metastasis.Conclusion Combination of ^(18)F-PSMA-1007 PET/CT imaging with pelvic MRI can provide added value of prostate cancer lesions,especially the former can detect multiple metastases,which is helpful for accurate preoperative evaluation of prostate cancer patients and guides clinicians to formulate reasonable Individualized treatment plan.
