摘 要:目的:探讨溃疡性结肠炎(UC)患者血清免疫球蛋白水平与肠道菌群构成变化及疾病严重程度的相关性。方法:选取104例UC患者为UC组;同期60名健康体检者为对照组。根据内镜严重指数(Baron评分)将UC组分为1~2分组(n=12)、2分组(n=41)和3分组(n=51);根据菌群失调分度将UC组分为菌群正常或Ⅰ度失调组(n=53)、Ⅱ度菌群失调组(n=25)和Ⅲ度菌群失调组(n=26);依照Mayo评分将UC组分为活动期组(n=65)和缓解期组(n=39)。根据活动度将UC活动期组分为轻度活动组(n=20)、中度活动组(n=27)、重度活动组(n=18)。比较不同分组对象的血清IgA、IgG、IgM水平,分析其与UC患者疾病活动度、内镜严重指数Baron评分和菌群失调度的关系。结果:UC组患者血清IgA、IgG、IgM水平低于对照组(P<0.05);活动期组患者血清IgA、IgG、IgM水平低于缓解期(P<0.05);不同内镜严重指数UC患者血清IgA、IgG、IgM水平比较:1~2分组>2分组>3分组(P<0.05);不同菌群失调分度UC患者血清IgA、IgM水平比较:菌群正常或Ⅰ度菌群失调组>Ⅱ度菌群失调组>Ⅲ度菌群失调组(P<0.05);不同疾病活动程度UC患者血清IgA、IgG、IgM水平比较:重度活动组<中度活动组<轻度活动组(P<0.05)。相关性分析显示,UC患者血清IgA、IgG、IgM水平与疾病活动度和内镜严重指数Baron评分均呈负相关(P<0.05);血清IgA、IgG水平与菌群失调程度均呈负相关(P<0.05)。结论:UC患者外周血免疫球蛋白水平低于健康人群,且随菌群失调加重、疾病活动度与内镜严重指数增加进一步降低。Objective:To investigate the correlation between serum immunoglobulin levels and changes in intestinal flora composition and disease severity in patients with ulcerative colitis(UC).Methods:104 patients with UC were selected as UC group.In the same period,60 healthy subjects were selected as the control group.According to the Baron score,UC group was divided into 1~2 group(n=12),2 group(n=41)and 3 group(n=51).The UC group was divided into normal flora or dysfunctionalⅠgroup(n=53),dysfunctionalⅡgroup(n=25)and dysfunctionalⅢgroup(n=26)according to the degree of dysbiosis.According to Mayo score,UC group was divided into active group(n=65)and remission group(n=39).The UC active group was divided into light activity group(n=20),moderate activity group(n=27)and heavy activity group(n=18)according to the activity level.The serum levels of IgA,IgG and IgM in patients in different groups were compared.The relationship with disease activity,Baron score of endoscopic severity index and bacterial dysscheduling in UC patients were analyzed.Results:The levels of serum IgA,IgG and IgM in UC group were lower than those in control group(P<0.05).The levels of serum IgA,IgG and IgM in active group were lower than those in remission stage(P<0.05).Comparison of the levels of serum IgA,IgG and IgM in UC patients with different endoscopic severity index:1~2 group>2 group>3 group(P<0.05).Comparison of the levels of serum IgA and IgM in UC patients with different microflora dysregulation:normal microflora andⅠmicroflora dysregulation group>Ⅱmicroflora dysregulation group>Ⅲmicroflora dysregulation group(P<0.05).Comparison of serum IgA,IgG and IgM levels in UC patients with different disease activity levels:severe activity group<moderate activity group<mild activity group(P<0.05).Correlation analysis showed that serum IgA,IgG and IgM levels were negatively correlated with disease activity and Baron score of endoscopic severity index in UC patients(P<0.05).The levels of IgA and IgG in serum were negatively correlated with the d
