APRF和CGF提高行上颌窦外提升种植术患者成骨能力的作用研究

摘　　要：目的:研究浓缩生长因子(CGF)和改良富血小板纤维蛋白(APRF)对进行上颌窦外提升种植术患者成骨能力的影响。方法:选择2021年1月—2022年1月到我院进行诊治的上颌磨牙缺失、进行上颌窦外提升种植术且RBH在2~4mm之间的患者20例,随机分为APRF组和CGF组,每组10例。两组患者均进行上颌窦外提升种植术,APRF组患者将APRF层剪碎混合Bio-Oss骨粉填入提升后的上颌窦窦底和上颌窦黏膜的骨板之间,CGF组患者将CGF层剪碎混合Bio-Oss骨粉填入提升后的上颌窦窦底和上颌窦黏膜的骨板之间。对患者的骨密度值(HU)、种植体边缘骨水平变化和存留率进行表征。结果:治疗前,APRF组和CGF组患者的骨密度值相似(P>0.05);术后2个月,两组植骨区骨密度值相对治疗前有明显升高(P<0.05);术后4个月,CGF组密度值高于APRF组,其中APRF组骨密度接近术前骨密度值(P>0.05);术后6个月,两组骨密度值相似(P>0.05),且APRF组与术前基本一致(P>0.05)。APRF组有1例患者种植体在2期修复时出现松动进行拔除,其余患者种植体均成功,APRF组种植体存留率为90%,CGF组种植体存留率为100%。术后2个月,两组患者种植体近中和远中边缘骨水平相似(P>0.05);术后4个月和6个月,APRF组患者种植体近远中边缘骨水平小于CGF组(P<0.05)。术后即刻,两组患者骨高度相似(P>0.05);术后2个月和4个月,APRF组患者的骨高度低于CGF组(P<0.05);但术后6个月,两组患者的骨高度基本一致(P>0.05)。术后2个月和4个月,CGF组患者的ISQ优于APRF组(P<0.05);但术后6个月,两组患者的ISQ基本一致(P>0.05)。结论:APRF和CGF应用于上颌窦外种植提升术患者均具有较好的成骨能力,其中CGF效果优于APRF。Objective:To study the effects of concentrated growth factors(CGF)and improved advanced platelet rich fibin(APRF)on the osteogenesis ability of patients undergoing external maxillary sinus enhancement.Methods:20 patients with maxillary molar loss who underwent external maxillary sinus lifting implantation and RBH between 2~4mm were selected from January 2021 to January 2022 and randomly divided into APRF group and CGF group,with 10 patients in each group.Patients in both groups underwent external maxillary sinus lifting implantation.Patients in the APRF group were filled between the bone plates of the maxillary sinus floor and the maxillary sinus mucosa after being lifted,and patients in the CGF group were filled between the bone plates of the maxillary sinus floor and the maxillary sinus mucosa after being lifted.Bone density values(HU),marginal bone level changes,and retention were characterized.Results:The bone mineral density of APRF group and CGF group was similar before treatment(P>0.05),and the bone mineral density of bone graft area was significantly increased at 2 months after surgery(P<0.05).4 months after surgery,the bone mineral density of CGF group was higher than that of APRF group,and the bone mineral density of APRF group was close to the preoperative bone mineral density(P<0.05).Bone mineral density was similar between the two groups 6 months after surgery(P>0.05),and the bone mineral density in the APRF group was basically the same as before surgery(P>0.05).In the APRF group,the implants of 1 patient were removed after loosening during the second stage of repair,and the implants of the other patients were successfully removed,with the implant retention rate of 90%in the APRF group and 100%in the CGF group.2 months after surgery,the medial and distal edge bone levels were similar between the two groups(P>0.05),the medial and distal edge bone levels in the APRF group were lower than those in the CGF group at 4 months and 6 months after surgery(P<0.05).Immediately after surgery,bone height was simi
