CSP、HSP与EMR技术治疗对结直肠小息肉患者完整切除率与出血率的影响对比

摘　　要：目的:对比结直肠小息肉患者在冷圈套器切除术(cold snare polypectomy,CSP)、热圈套器切除术(hot snare polypect-omy,HSP)、内镜下黏膜切除术(Endoscopic mucosal resection,EMR)技术治疗下,其息肉完整切除率、出血率所受影响。方法:在本院胃肠科手术室2022年2月~2023年2月入院行手术治疗结直肠小息肉患者中选出85例(30例CSP技术,25例HSP技术,30例EMR技术),对比三种治疗技术下患者息肉完整切除率、出血率、临床时间指标、CRP指标以及BBPS评分。结果:CSP技术、HSP技术、EMR技术下患者息肉完整切除率分别为90.00%、92.00%、96.67%,BBPS评分分别为(7.50±1.13)分、(7.81±1.14)分、(7.71±0.85)分,组间数据对比无统计学差异(P>0.05);CRP指标分别为(7.40±1.11)pmol/L、(10.38±1.23)pmol/L、(11.02±1.46)pmol/L,出血率分别为3.33%、4.00%、23.33%,EMR技术出血率较高,且CRP指标、临床时间指标CSP技术数据略小,组间对比存在显著差异(P<0.05)。结论:CSP技术、HSP技术、EMR技术在结直肠小息肉治疗中均能表现出完整切除息肉优势,但EMR技术相比较容易出现术中出血现象,且CSP技术具有时效性强、炎症反应小特征,可以优先考虑患者在CSP技术中的适用性。Objective:To compare the effects of CSP(Cold Trap Resection),HSP(Hot Trap Resection),and EMR(Endoscopic Mucosectomy)techniques on the complete polypectomy rate and bleeding rate of patients with small colorectal polyps.Methods:85 patients(30 cases of CSP technique,25 cases of HSP technique,and 30 cases of EMR technique)were selected for surgical treatment of small colorectal polyps in the gastrointestinal operating room of our hospital from February 2022 to February 2023.The complete polypectomy rate,bleeding rate,clinical time interval indicators,CRP indicators,and BBPS score of the patients were compared under three treatment techniques.Result:The complete resection rates of polyps in patients using CSP,HSP,and EMR techniques were 90.00%,92.00%,and 96.67%,respectively.The BBPS scores were(7.50±1.13)points,(7.81±1.14)points,and(7.71±0.85)points,and there was no statistically significant difference between the groups(P>0.05);CRP indicators(7.40±1.11)pmol/L,(10.38±1.23)pmol/L,and(11.02±1.46)pmol/L,with bleeding rates of 3.33%,4.00%,and 23.33%.EMR technology has a higher bleeding rate,and CRP indicators and clinical time indicators CSP technology data are slightly small,with significant differences between groups(P<0.05).Conclusion:CSP technology,HSP technology,and EMR technology can all demonstrate the advantage of complete polypectomy in the treatment of small colorectal polyps.However,EMR technology is more prone to intraoperative bleeding,and CSP technology has strong timeliness and small inflammatory response characteristics.Therefore,the applicability of patients in CSP technology can be prioritized.
