摘 要:目的:探究超声肝动脉血流参数对肝硬化患者临床分期的诊断价值。方法:选取2021年3月至2022年3月就诊的102例肝硬化患者为研究对象并将其纳入观察组,将同期于医院进行体检的102例健康人群纳入对照组,根据相关指南将观察组患者分为代偿期组与非代偿期组,采用彩色多普勒超声检测各组受试者血流频谱、肝动脉内径,记录超声肝动脉相关血流参数并计算各参数曲线下面积值(AUC)值,分析其诊断价值。结果:观察组患者肝动脉内径、肝动脉血流量、平均流速、最高流速及最低流速均高于对照组(P<0.05),两组受试者阻力指数(RI)比较差异无统计学意义(P>0.05)。失代偿期组患者肝动脉内径、肝动脉血流量、平均流速、最高流速及最低流速均高于代偿期组(P<0.05),两组患者RI比较差异无统计学意义(P>0.05)。超声肝动脉血流参数:肝动脉内径、肝动脉血流量、平均流速、最高流速、最低流速诊断鉴别不同临床分期的肝硬化时,以肝动脉血流量的AUC值最高,敏感度、特异度分别为97.44%与100%,其AUC值与肝动脉内径、平均流速、最高流速、最低流速比较差异有统计学意义(P<0.05)。结论:超声肝动脉血流参数特别是肝动脉血流量对于肝硬化患者临床分期具有较高的诊断价值,临床上可采用超声肝动脉血流参数作为诊断肝硬化患者临床分期的依据。Objective:To explore the value of hepatic artery blood flow parameters measured by ultrasound in clinical staging of patients with liver cirrhosis.Methods:A total of 102 patients with liver cirrhosis admitted to the hospital from March 2021 to March 2022 were selected as the observation group,and 102 healthy people who received physical examination in the hospital during the same period were selected as the control group.Patients in the observation group were divided into the compensated group and the decompensated group according to relevant guidelines.Blood flow spectrum and inner diameter of hepatic artery were measured by color Doppler ultrasound.The relevant blood flow parameters of hepatic artery were recorded and the area under the curve(AUC)of each parameter was calculated to analyze its diagnostic value.Results:The inner diameter of hepatic artery,hepatic artery blood flow,mean velocity,maximum velocity and minimum velocity in the observation group were higher than those in the control group(P<0.05),but there was no significant difference in resistance index(RI)between the groups(P>0.05).The inner diameter of hepatic artery,hepatic artery blood flow,mean velocity,maximum velocity and minimum velocity in the decompensated group were higher than those in the compensated group(P<0.05),but there was no significant difference in RI between the two groups(P>0.05).Ultrasound hepatic artery blood flow parameters:Hepatic artery diameter,hepatic artery blood flow,average flow velocity,maximum flow velocity and minimum flow velocity in the diagnosis of different clinical stages of liver cirrhosis,hepatic artery blood flow had the highest AUC value,the sensitivity and specificity were 97.44%and 100%,respectively.The AUC value was significantly different from hepatic artery diameter,average flow velocity,maximum flow velocity and minimum flow velocity(P<0.05).Conclusion:Ultrasonic hepatic artery blood flow parameters,especially hepatic artery blood flow,have high diagnostic value for the clinical staging of patients
