电子耳蜗植入术患儿父母焦虑程度及其影响因素分析

摘　　要：目的观察行电子耳蜗植入术患儿父母的焦虑情况及影响因素分析。方法96位择期行电子耳蜗植入术患儿的父母在术前和术后24 h内完成中文版焦虑自测量表。除社会人口学信息,还调查了患儿父母是否担心手术和麻醉相关的风险。结果有22.9%行电子耳蜗植入术患儿的父母存在术前焦虑。患儿母亲的术前焦虑程度高于患儿父亲(44.2 vs.40.3,P=0.042),患儿母亲(36.2 vs.44.2,P<0.001)和父亲(35.1 vs.40.3,P=0.004)术后焦虑程度均较术前显著下降。术后焦虑发生率为2.1%,术后患儿母亲和父亲之间的焦虑程度差异无统计学意义(36.2 vs.35.1,P=0.460)。与患儿父母术前焦虑相关的因素包括:患儿为男性、居住地非江浙沪、患儿行双侧电子耳蜗植入术及父母担心麻醉相关的风险。结论先天性耳聋的患儿父母存在术前焦虑,各种因素会影响术前焦虑水平。麻醉医师可以在术前为患儿父母提供充分的与麻醉相关的信息,降低其对麻醉风险的担忧。Objective To investigate anxiety levels in parents of cochlear implantation patients and to identify factors that affect parental anxiety.Methods Ninety-six parents of children undergoing cochlear implantation at otologic center completed the Chinese version of the self-rating anxiety scale before and 24 hours after surgery.They also answered questions about their socio-demographic situation,their concern about the complications of surgery and anesthesia.Results 22.9%of the parents had pre-operative anxiety.In parents of deaf children,mothers had higher level of anxiety compared with fathers preoperatively(44.2 vs.40.3,P=0.042).Anxiety levels both decreased postoperatively in mothers(36.2 vs.44.2,P<0.001)and fathers(35.1 vs.40.3,P=0.004)comparing with preoperative levels.The incidence rate of postoperative anxiety was 2.1%.The postoperative anxiety level was similar between mothers and fathers(36.2 vs.35.1,P=0.460).Factors associated with preoperative anxiety were male child,living outside Zhejiang Province,Jiangsu Province and Shanghai,undergoing bilateral surgery,and worrying about the complication of anesthesia.Conclusion Parents of children with congenital deafness have high level of preoperative anxiety which is affected by various factors.Detailed information about anesthesia should be provided preoperatively to make less anxiety for parents.
