全腹腔镜下远端胃癌根治术对早中期胃癌患者胃肠道屏障功能及肿瘤微转移指标的影响

摘　　要：目的 探讨全腹腔镜下远端胃癌根治术对早中期胃癌患者胃肠道屏障功能及肿瘤微转移指标的影响。方法 选取2017年2月至2021年3月收治的120例早中期胃癌患者为研究对象,按照随机数字表法将其分为对照组与观察组,各60例。对照组行传统开腹术治疗,观察组行全腹腔镜下远端胃癌根治术治疗。比较两组的治疗效果。结果 观察组的术中出血量少于对照组,排气时间及住院时间短于对照组(P<0.05)。术后1 d,观察组的二胺氧化酶(DAO)、D-乳酸水平低于对照组(P<0.05)。术后1 d,观察组的核因子κB(NF-κB)、白细胞介素-6(IL-6)、白细胞介素-17(IL-17)水平低于对照组(P<0.05)。手术结束时,观察组的多巴脱羧酶(DDC)、癌胚抗原(CEA)水平低于对照组(P<0.05)。结论 全腹腔镜下远端胃癌根治术治疗早中期胃癌的效果较好,可有效保护胃肠道屏障功能,降低炎症反应程度,减少肿瘤微转移,值得推广。Objective To explore the effects of total laparoscopic distal gastrectomy on gastrointestinal barrier function and tumor micrometastasis indexes in patients with early and middle stage gastric cancer.Methods A total of 120 patients with early and middle stage gastric cancer admitted from February 2017 to March 2021 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 60 cases in each group.The control group was treated with traditional laparotomy,and the observation group was treated with total laparoscopic distal gastrectomy.The therapeutic effects of the two groups were compared.Results The intraoperative blood loss in the observation group was less than that in the control group,and the exhaust time and hospitalization time were shorter than those in the control group(P<0.05).At 1 d after operation,the levels of diamine oxidase(DAO)and D-lactic acid in the observation group were lower than those in the control group(P<0.05).At 1 d after operation,the levels of nuclear factor kappa-B(NF-κB),interleukin-6(IL-6)and interleukin-17(IL-17)in the observation group were lower than those in the control group(P<0.05).At the end of operation,the levels of dopa decarboxylase(DDC)and carcinoembryonic antigen(CEA)in the observation group were lower than those in the control group(P<0.05).Conclusion Total laparoscopic distal gastrectomy in the treatment of early and middle gastric cancer has good effect.It can effectively protect the gastrointestinal barrier function,reduce the degree of inflammatory response,and reduce tumor micrometastasis.It is worthy of promotion.
