甲状腺微小乳头状癌患者中央区淋巴结清扫及甲状旁腺功能保护的研究进展

摘　　要：随着人民健康意识的增强及超声影响技术的发展,甲状腺微小乳头状癌的发病率逐年增长,目前我国专家共识仍推荐对临床淋巴结阴性(Clinical negative lymph nodes,cN0)的甲状腺微小乳头状癌(Papillary thyroid microcarcinoma,PTMC)患者行预防性中央区淋巴结清扫,在甲状腺手术当中,对甲状旁腺功能的保护是目前一大难点及热点问题。本文从国内外的研究现状出发,总结近年来有关甲状腺癌的淋巴结清扫问题、甲状旁腺功能保护、针对术后甲状旁腺功能减退的补钙策略等问题,以期减少甲状腺癌术后并发症的发生率,提高患者生活质量及其对医疗的满意度,同时也为临床医师提供新的诊疗思路。With the improvements in people's health awareness and the continuous development of ultrasonic impact technology,it was found that thyroid papillary carcinoma incidence increased year by year.Currently,the consensus among experts in China still recommends to the patients with PTMC cN0 by preventive cleaning of lymph node in central region in thyroid surgery.During the operation,the protection of parathyroid function is a big difficulty and a hotspot.This paper,based on the domestic and foreign research progress,summarizes the cleaning of lymph node of thyroid carcinoma in recent years,parathyroid function protection,postoperative parathyroid function impairment of calcium strategy,in order to reduce the incidence of postoperative complications of thyroid,improve the patient's quality of life and satisfaction of medical treatment,provide clinicians with a new ideas in diagnosis and treatment.
