重复经颅磁刺激联合抗抑郁药物在首发抑郁症中的疗效研究

摘　　要：目的探讨重复经颅磁刺激联合抗抑郁药物在首发抑郁症中的治疗疗效。方法选取首发抑郁症患者89例,随机分组为研究组(44例)和对照组(45例),研究组在服用草酸艾司西酞普兰的基础上辅以左前额叶背外侧区(L-DLPFC)高频重复经颅磁刺激物理治疗,对照组则在服用草酸艾司西酞普兰的基础上辅以高频重复经颅磁伪刺激治疗,于治疗前及治疗后第2、4周末采用汉密尔顿抑郁量表(HAMD)、贝克抑郁量表(BDI)评估两组患者抑郁减分情况并进行统计分析。结果治疗后第2、4周末,研究组HAMD减分疗效高于对照组(P<0.05)。两组治疗后第2、4周末HAMD、BDI评分均较治疗前降低(P<0.01)。治疗后第2、4周末研究组HAMD、BDI评分均低于对照组(P<0.05)。结论药物联合重复经颅磁刺激治疗较单纯药物治疗可显著降低首发抑郁症患者HAMD、BDI评分并增强治疗效果。Objective To explore the therapeutic effects of repetitive transcranial magnetic stimulation(rTMS)therapy combined with antidepressants on the patients with first-episode depression.Methods A total of 89 patients with first-episode depression were selected and randomly divided into study group(44 cases)treated with high frequency rTMS at left dorsolateral prefrontal cortex(L-DLPFC)on the basis of escitalopram and control group(45 cases)treated with sham high frequency rTMS on the basis of escitalopram.To assess the depressive symptoms with Hamilton Depression Scale(HAMD)and Beck Depression Inventory(BDI)before treatment and at the 2 nd,4 th weekends after treatment for statistical analysis.Results At the 2 nd,4 th weekends after treatment,the treatment effective rates in study group were significantly higher than those in control group(P<0.05);and the total scores of HAMD and BDI in both groups significantly decreased compared with those before treatment(P<0.01).At the 2 nd,4 th weekends after treatment,the total scores of HAMD and BDI in study group were all significantly lower than those in control group(P<0.05).Conclusion The combination of antidepressants and rTMS therapy can not only significantly reduce HAMD and BDI scores in patients with first-episode depression,but also improve the therapeutic effects.
