真武汤联合穴位敷贴治疗原发性肝癌合并腹水的效果

摘　　要：目的探讨真武汤联合穴位敷贴治疗原发性肝癌合并腹水患者的临床效果。方法选取2020年5月—2021年11月福建省南平市人民医院收治的110例原发性肝癌合并腹水患者,采用随机数字表法分为对照组与观察组,每组55例。对照组患者采用常规西医治疗,观察组患者在西医治疗基础上采用真武汤联合穴位敷贴进行治疗。比较2组患者治疗前后腹水改善情况及腹水最大液平直径变化情况,同时采用Karnofsky评分评估2组患者生活质量,统计2组患者不良反应发生情况。结果观察组患者治疗后腹水改善总有效率为85.45%(47/55),高于对照组的52.72%(29/55),差异有统计学意义(χ^(2)=13.793,P<0.001)。2组患者治疗前腹水最大液平直径比较,差异无统计学意义(P>0.05);2组患者治疗后腹水最大液平直径均小于治疗前,且观察组患者治疗后腹水最大液平直径小于对照组,差异均有统计学意义(P<0.05)。2组患者治疗前Karnofsky评分比较,差异无统计学意义(P>0.05);2组患者治疗后Karnofsky评分均高于治疗前,且观察组患者治疗后Karnofsky评分高于对照组,差异均有统计学意义(P<0.05)。观察组患者白细胞减少、红细胞减少、恶心呕吐及腹泻发生率均低于对照组,差异均有统计学意义(P<0.05)。结论真武汤联合穴位敷贴可有效改善肝癌腹水患者临床症状,提高患者生活质量,且不良反应发生风险较低,值得在临床推广。Objective This paper aims to investigate the clinical effect of Zhenwu decoction combined with acupoint application in the treatment of primary liver cancer patients with ascites.Methods From May 2020 to November 2021,110 patients with primary liver cancer with ascites were randomly assigned to the control group(n=55)and observation group(n=55).Patients in the control group received conventional Western medicine treatment;patients in the observation group received Zhenwu decoction combined with acupoint application on the basis of the conventional treatment.The improvement of ascites and the change in the maximum fluid level diameter of ascites before and after treatment were compared between the two groups.Karnofsky score was used to assess the quality of life of the two groups,and the incidence of adverse reactions in the two groups was statistically analyzed.Results The overall response rate of ascites improvement after treatment in the observation group was 85.45%(47/55),which was significantly higher than that in the control group(52.72%,29/55;χ^(2)=13.793,P<0.001).There was no significant difference in the maximum fluid level diameter of ascites before treatment between the two groups(P>0.05).The maximum fluid level diameter of ascites after treatment in the two groups was significantly decreased,and the maximum fluid level diameter of ascites after treatment in the observation group was significantly smaller than that in the control group(P<0.05).There was no significant difference in Karnofsky score before treatment between the two groups(P>0.05).The Karnofsky score after treatment in both groups was significantly increased,and the Karnofsky score after treatment in the observation group was significantly higher than that in the control group(P<0.05).The incidence of leukopenia,erythropenia,nausea,vomiting,and diarrhea in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Zhenwu decoction combined with acupoint application can effectively improve the clinical
