体液免疫检测与细胞免疫检测在脓毒症患者诊断中的价值比较

摘　　要：目的:分析体液免疫检测与细胞免疫检测在脓毒症患者诊断中的价值。方法:选取2021年1月—2022年12月常熟市三家三级医院诊治的脓毒症患者100例为研究组,同期体检的健康者64例为对照组。均接受体液免疫与细胞免疫检测。抽血测定CD3^(+)、CD4^(+)、CD8^(+)表达及免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫球蛋白A(IgA)水平,比较两组各项体液免疫检测与细胞免疫检测指标,分析脓毒症患者各项体液免疫检测指标与细胞免疫检测指标的关系。结果:对照组血清IgG、IgM、IgA水平及CD4^(+)、CD3^(+)的表达均高于研究组,差异有统计学意义(P<0.001);对照组与研究组血清CD8^(+)的表达比较,差异无统计学意义(P>0.05);血清IgG、IgM、IgA水平与CD3^(+)表达呈正相关,差异有统计学意义(P<0.05);血清IgG、IgM、IgA水平与CD4^(+)表达呈正相关,差异有统计学意义(P<0.05)。结论:体液免疫检测与细胞免疫检测指标对于脓毒症的早期诊断均有一定的应用价值,并且两种免疫检查方式之间存在密切的联系,在实际应用中可相互参考、借鉴,进一步提高诊断价值。Objective:To analyze the value of humoral immune detection and cellular immune detection in the diagnosis of patients with sepsis.Methods:From January 2021 to December 2022,one hundred patients with sepsis who were diagnosed and treated in three tertiary hospitals in Changshu City were selected as the research group,and sixty-four healthy people who underwent physical examination during the same period were selected as the control group.Both humoral immunity and cellular immunity were tested.Blood was drawn to measure the expression of CD3^(+),CD4^(+),CD8^(+)and the levels of immunoglobulin G(IgG),immunoglobulin M(IgM)and immunoglobulin A(IgA).The humoral immunity detection and cellular immunity detection indexes of the two groups were compared,and the relationship between the humoral immunity detection indexes and the cellular immunity detection indexes of patients with sepsis were analyzed.Results:The levels of serum IgG,IgM,IgA and the expressions of CD4^(+)and CD3^(+)in the control group were higher than those in the research group,and the difference was statistically significant(P<0.001);There was no significant difference in the expression of serum CD8^(+)between the control group and the research group(P>0.05);Serum IgG,IgM,and IgA levels were positively correlated with CD3^(+)expression,and the difference was statistically significant(P<0.05);Serum IgG,IgM,IgA levels were positively correlated with CD4^(+)expression,and the difference was statistically significant(P<0.05).Conclusion:Both humoral immune detection and cellular immune detection indicators have certain application value for the early diagnosis of sepsis,and there is a close relationship between the two immune detection methods,which can be used for mutual reference and reference in practical applications to further improve the diagnosis value.
