后颅窝天幕夹层之间巨大表皮样囊肿1例报告并文献复习

摘　　要：目的 探讨后颅窝天幕夹层之间表皮样囊肿的诊断和治疗策略。方法 回顾性分析1例后颅窝天幕夹层之间巨大表皮样囊肿的临床资料,采用左侧跨幕上下Poppen入路,显微镜下切除。结果 术后病理证实为后颅窝表皮样囊肿并肉芽肿性炎。患者术后恢复好,入院前的相关症状消失。结论 后颅窝天幕夹层之间巨大表皮样囊肿极罕见,该例临床及影像学表现不典型,术前诊断困难,主要治疗方法仍以手术切除为主。Objective To investigate the diagnosis and treatment of giant epidermoid cyst in the tentorium cerebellum of posterior cranial fossa.Methods The clinical data of 1 patient with giant epidermoid cyst in the tentorium cerebellum of posterior cranial fossa was analyzed retrospectively.The left upper and lower transentural Poppen approach was used to remove the cyst under microscope.Results Postoperative pathology confirmed the posterior cranial fossa epidermoid cyst with granulomatous inflammation.The patient recovered well after the operation,and the related symptoms disappeared before admission.Conclusions The giant epidermoid cyst of the posterior cranial fossa tentorium dissection is extremely rare.The clinical and imaging manifestations of this case are atypical and the preoperative diagnosis is difficult.The main treatment is still surgical resection.
