摘　　要：目的分析小剂量rt-PA联合阿加曲班治疗急性脑梗死的疗效及对脑组织影像学指标的影响。方法选取2021-01—2022-12铜陵市人民医院收治的88例急性脑梗死患者为研究对象,按随机数字表法分为观察组和对照组,观察组44例予以基础治疗+0.6 mg/kg的小剂量rt-PA+阿加曲班治疗,对照组44例予以基础治疗+0.9 mg/kg标准剂量rt-PA治疗,比较2组患者的临床疗效、脑组织影像学指标、神经功能与肢体功能及不良反应。结果观察组临床治疗总有效率97.73%,高于对照组的84.09%(P<0.05)。观察组治疗后ADC水平、FMA评分高于对照组(P<0.05),FA水平、NIHSS评分低于对照组(P<0.05)。观察组总不良反应发生率4.55%,低于对照组的18.18%(P<0.05)。结论小剂量rt-PA联合阿加曲班治疗急性脑梗死能够改善患者脑组织影像学指标、神经功能与肢体功能,不良反应发生率低,临床疗效更加显著。Objective To explore the clinical efficacy of low-dose rt-PA combined with argatroban anhydrous in acute cerebral infarction and its effect on imaging indexes of brain tissue.Methods From January 2021 to December 2022,88 patients with acute cerebral infarction admitted to the Tongling People’s Hospital were selected as the study objects and were divided into observation group and control group according to the random number table method,44 patients in the observation group were treated with basic treatment+0.6 mg/kg low-dose rt-PA+argatroban anhydrous treatment,and 44 patients in the control group were treated with basic treatment+0.9 mg/kg standard dose rt-PA.The clinical efficacy,brain tissue imaging indicators,neurological function and limb function and adverse reactions were compared.Results The total clinical response rate of patients in the observation group was 97.73%,which was higher than that(84.09%)in the control group was(P<0.05),the ADC and FMA scores were higher than those in the control group(P<0.05),FA and NIHSS scores were lower than those in the control group(P<0.05),and the incidence of total adverse reactions was 4.55%,which was lower than that(18.18%)in the control group(P<0.05).Conclusion Low-dose of rt-PA combined with argatroban anhydrous for acute cerebral infarction can improve the imaging index of brain tissue,neurological function and limb function of patients,the incidence of adverse reactions is low,and the clinical effect is more significant.
