基于“伏邪学说”探析国医大师王琦“脱敏消癜汤”的组方规律

摘　　要：过敏性紫癜(HSP)是一种病程长易反复发作、迁延难愈的血管变态反应性疾病,属于中医“肌衄”“斑毒”“葡萄疫”范畴。该病的发病特点与中医“伏邪致病”病机基本一致,中国工程院院士、国医大师王琦教授“辨西医之名,融中医之论”,认为过敏体质是HSP发生背景和土壤,临床上采用“辨体-辨病-辨证”的诊疗模式和“主病主方”的思路自拟“脱敏消癜汤”,临床卓有成效。故文章从伏邪学说角度探析“脱敏消癜汤”的组方规律,为临床防治HSP提供参考。Henoch-Schönlein purpura(HSP)is a kind of vascular allergic disease with long course and recurrence,which is difficult to be cured,and belongs to the category of‘epistaxis’,‘spot poison’and‘grape disease’in traditional Chinese medicine(TCM).The pathogenetic characteristics of HSP are basically consistent with the pathogenic mechanism of latent pathogen in TCM.TCM master WANG Qi,academician of the Chinese academy of engineering,believes that allergy constitution is the background of HSP,and draws up‘Tuomin Xiaodian Decoction’based on the treatment model of‘differentiation-constitution,differentiation-disease,differentiation-syndrome’and the idea of‘the main formula for the main disease’,which has remarkable clinical effects.Therefore,this paper aims to explore the formula of‘Tuomin Xiaodian Decoction’from the perspective of latent pathogen to provide a reference for clinical treatment of HSP and other allergic diseases.
