摘 要:目的探讨三维重建技术在神经外科的颅底手术教学中的应用效果。方法纳入2020年1月—2021年12月在西安交通大学第一附属医院神经外科颅底肿瘤中心轮转的培训人员44名作为研究对象,包括在读博士研究生、硕士研究生、进修医师,分为对照组和观察组,各22名。对照组采用传统教学方法,以幻灯片讲解为主的授课方式;观察组在传统教学方法基础上加入了三维重建可视化教学内容,重现肿瘤周围重要的神经血管解剖结构,模拟手术入路。授课结束后,对两组学员进行评价。分为客观(理论考试)及主观(自我评价)两部分。结果第一部分理论考试成绩观察组明显优于对照组,平均成绩分别为(78.5±8.2)分和(69.3±9.6)分,差异有统计学意义(P<0.05)。分层分析后发现观察组中不及格1名,及格8名,良好9名,优秀4名;对照组中不及格3名,及格15名,良好4名,优秀0名,差异有统计学意义(P<0.05)。第二部分学员自我评价,观察组成员认为其收获也明显大于对照组,观察组中自我评价为无收获0名,有所收获10名,收获很大12名;对照组中自我评价为无收获6名,有所收获14名,收获很大2名,差异有统计学意义(P<0.05)。结论利用患者术前的影像资料进行三维重建并将该技术用于年轻医师的颅底手术教学培训中,极大程度地提高了颅底手术教学的成效。Objective To explore the application effect of three-dimensional reconstruction technology in the teaching of skull base surgery in neurosurgery.Methods A total of 44 trainees,including doctoral students,master students and advanced doctors,who were rotated in the neurosurgery skull base tumor center of the First Affiliated Hospital of Xi'an Jiaotong University from January 2020 to December 2021,were randomly divided into control group and observation group,with 22 trainees in each group.In the control group,the traditional teaching method was used,and the teaching method was mainly slide presentation.On the basis of traditional teaching methods,the visualization teaching content of three-dimensional reconstruction was added to the observation group to reproduce the important neurovascular anatomical structures around the tumor and simulate the surgical approach.After the lecture,the two groups of students were evaluated.It was divided into objective(theory examination)and subjective(self-evaluation)two parts.Results In the first part,the observation group was significantly better than the control group,with an average score of(78.5±8.2)points and(69.3±9.6)points respectively,with statistical significance(P<0.05).After stratified analysis,it was found that in the observation group,1 participant failed,8 participants passed,9 participants were good and 4 participants were excellent.In the control group,3 participants failed,15 participants passed,4 participants were good and 0 were excellent,and the difference was statistically significant(P<0.05).In the second part,the students self-evaluated,and the members of the observation group thought that their gains were also significantly greater than those of the control group.In the observation group,0 participants had no harvest,10 participants had some harvest,and 12 participants had great harvest.In the control group,2 participants had no harvest,14 participants had some harvest,and 6 participants had great harvest,and the difference was statistically significant(
