COX-2、MSS表达与结直肠癌临床病理特征及其预后的相关性研究

摘　　要：目的探讨结直肠癌环氧化酶-2(COX-2)、微卫星稳定性(MSS)表达与其临床病理特征及预后的相关性。方法选取64例结直肠癌患者。采集所有患者的病灶组织新鲜样本(病灶组)与正常肠黏膜(距癌组织≥5 cm)新鲜标本(对照组),采用免疫组化法,检测两组的COX-2、MSS表达情况,并分析COX-2、MSS表达与患者临床病理特征及预后的关系。结果病灶组COX-2、MSS阳性表达率高于对照组,有统计学差异(P<0.05);结直肠癌患者的COX-2、MSS阳性表达与患者年龄、性别无关(P>0.05),与肿瘤分化程度、淋巴结转移、临床分期以及肿瘤直径密切相关(P<0.05);Kaplan-Meier结果显示:COX-2、MSS阳性表达患者3年生存率低于COX-2、MSS阴性表达患者,有统计学差异(P<0.05)。结论结直肠癌患者癌组织内的COX-2、MSS呈现出高表达状态,且与肿瘤分化程度、淋巴结转移、临床分期以及肿瘤直径密切相关,COX-2、MSS阳性表达的患者3年生存率更低,预后更差,临床需予以积极重视。Objective To investigate the correlation between the expression of cyclooxygenase-2(COX-2)and microsatellite stability(MSS)in colorectal cancer and their clinical pathological characteristics and prognosis.Methods 64 patients with colorectal cancer were selected.Collect fresh samples of lesion tissue(lesion group)from all patients and fresh samples of normal intestinal mucosa(≥5 cm away from cancer tissue)(control group).Use immunohistochemistry to detect the expression of COX-2 and MSS in both groups,and analyze the relationship between COX-2 and MSS expression and clinical pathological characteristics and prognosis of patients.Results The positive expression rates of COX-2 and MSS in the lesion group were higher than those in the control group,with statistical differences(P<0.05);The positive expression of COX-2 and MSS in colorectal cancer patients is not related to age and gender(P>0.05),but is closely related to tumor differentiation,lymph node metastasis,clinical staging,and tumor diameter(P<0.05);The Kaplan Meier results showed that the 3-year survival rate of COX-2 and MSS positive expression patients was lower than that of COX-2 and MSS negative expression patients,with statistical differences(P<0.05).Conclusion COX-2 and MSS are highly expressed in colorectal cancer tissues,and are closely related to tumor differentiation,lymph node metastasis,clinical staging,and tumor diameter.Patients with positive expression of COX-2 and MSS have a lower 3-year survival rate and poorer prognosis,which should be highly valued in clinical practice.
