双平面钼靶定位技术在乳腺钙化灶切除术中的临床应用价值

摘　　要：目的探讨乳腺钼靶CC位(头尾位)和MLO位(内外侧斜位)双平面摄片模式下精准切除乳腺钙化灶的临床效果。方法选取60例经乳腺钼靶摄片提示BI-RADS 4~5级的患者,均采用双平面钼靶定位技术行乳腺钙化灶定位,并完成钙化灶切除手术,分析临床效果。结果60例乳腺钙化灶患者均采用双平面钼靶定位技术定位成功,均完整切除乳腺钙化灶。在60例乳腺钙化灶患者中发现乳腺癌23例(浸润性癌5例,导管原位癌伴微小浸润6例,导管原位癌12例),发现良性病变37例(乳腺非典型增生6例,导管内乳头状瘤2例,乳腺腺病29例)。结论在乳腺钼靶CC位(头尾位)和MLO位(内外侧斜位)双平面摄片模式下采用乳腺定位针精准定位乳腺钙化灶可以精准切除乳腺钙化灶,对于没有立体定位系统的钼靶机的基层医院具有很好的临床应用价值。Objective To investigate the clinical effect of this study in the double plane radiography mode of breast molybdenum target CC(craniocaudal)and MLO(mediolateral oblique)respectively to accurately locate calcification of breast,so as to achieve the purpose of accurate resection of breast calcification focus.Methods A total of 60 patients with BI-RADS grade 4-5 indicated by Mammary gland molybdenum target were selected.The calcification of breast were located by double plane radiography,and the calcification resection of breast was completed.The clinical effect was analyzed.Results 60 cases of breast calcification were successfully located by double-plane molybdenum target technique,and all of them were completely resected.In 60 cases of breast calcification,23 cases of breast cancer(5 cases of invasive carcinoma,6 cases of ductal carcinoma in situ with microinvasion,12 cases of ductal carcinoma in situ)and 37 cases of benign lesions(6 cases of atypical hyperplasia,2 cases of intraductal papilloma,29 cases of breast adenosis)were found.Conclusion In the double plane radiography mode of breast molybdenum target CC(craniocaudal)and MLO(mediolateral oblique),the precise location of breast calcification focus can be accurately removed by the breast positioning needle,which has good clinical application value for primary hospital without Molybdenum target machine for stereopositioning system.
