腹腔镜辅助阴式广泛全子宫切除联合淋巴切除术对宫颈癌患者盆底肌功能及免疫功能的影响

摘　　要：目的研究腹腔镜辅助阴式广泛全子宫切除联合淋巴切除术对宫颈癌患者盆底肌功能及免疫功能的影响。方法回顾性分析116例宫颈癌患者临床资料,根据手术方案不同分成腹腔镜组61例、开腹组55例。腹腔镜组行腹腔镜辅助阴式广泛全子宫切除+淋巴切除术联合治疗,开腹组行常规的开腹广泛子宫切除术+淋巴结清扫术联合治疗,术后对患者进行12个月随访。对比2组患者术后临床疗效、复发率、盆底肌功能、免疫功能及并发症发生率。结果腹腔镜组患者治疗总有效率(80.33%)较开腹组(61.82%)更高,术后复发率(21.31%)较开腹组(43.64%)更低。术后3 d盆底肌功能测定正常率(81.97%)较开腹组(65.45%)更高,差异有统计学意义(P<0.05)。相较于术前,2组患者术后3 d的免疫功能指标CD3^(+)与CD4^(+)、CD8^(+)水平均降低,但腹腔镜组高于开腹组,差异有统计学意义(P<0.05)。腹腔镜组患者术后并发症总发生率(8.20%)低于开腹组(21.82%),差异有统计学意义(P<0.05)。结论腹腔镜辅助阴式广泛全子宫切除+淋巴切除术联合治疗宫颈癌的疗效确切,能减少对盆底肌功能和免疫功能的影响,减少并发症的发生,预防疾病复发。Objective To study the effect of laparoscopic assisted vaginal hysterectomy combined with lymphadenectomy on pelvic floor muscle function and immune function in patients with cervical cancer.Methods The clinical data of 116 patients with cervical cancer who underwent surgical treatment were reviewed.According to different surgical schemes,they were divided into 61 cases in laparoscopy group and 55 cases in laparotomy group.The laparoscopic group was treated with laparoscopic assisted vaginal extensive hysterectomy+lymphadenectomy,and the open group was treated with conventional open extensive hysterectomy+lymph node dissection.The patients were followed up for 12 months.The postoperative clinical efficacy,recurrence rate,pelvic floor muscle function,immune function and complication rate were compared between the 2 groups.Results The total effective rate of laparoscopic group(80.33%)was higher than that of laparotomy group(61.82%),the postoperative recurrence rate(21.31%)was lower than that of laparotomy group(43.64%),and the normal rate of pelvic floor muscle function three days after operation(81.97%)was higher than that of laparotomy group(65.45%)(P<0.05);Compared with before operation,the levels of immune function indexes CD3^(+),CD4^(+),CD8^(+)in the 2 groups were lower 3 days after operation,but the level in the laparoscopic group was higher than that in the open group(P<0.05);The total incidence of postoperative complications in laparoscopy group(8.20%)was lower than that in laparotomy group(21.82%).The difference was statistically significant(P<0.05).Conclusion Laparoscopic assisted vaginal hysterectomy combined with lymphadenectomy is effective in the treatment of cervical cancer.It can reduce the impact on pelvic floor muscle function and immune function,reduce the occurrence of complications and prevent disease recurrence.It is worth advocating.
