呼吸肌锻炼对进展期帕金森病患者生活质量的影响

摘　　要：目的观察呼吸肌功能锻炼对进展期帕金森病患者咳嗽、吞咽功能和生活质量的影响。方法存在吞咽困难的帕金森病患者37例,随机分为2组,对照组进行常规治疗,治疗组除常规治疗外,加用呼吸肌功能锻炼;治疗30 d后分别检测2组患者的咳嗽功能、吞咽功能和生活质量,检测指标包括吸气相时间(Inspiration phase duration,IPD)、压缩相时间(Compression phase duration,CPD)、呼气相时间(Expiratory phase rise time,EPRT)以及咳嗽容积加速(Volume acceleration,VA)值;吞咽功能评分采用穿透/误吸(Penetration/Aspiration,P/A)值表示,生活质量采用生活质量评分(Activity of daily life,ADL)进行评估,并进行统一帕金森病量表(Unified Parkinson disease rating scale,UPDRS)评分。结果与对照组比较,治疗组的IPD,CPD和EPRT时程显著缩短,咳嗽VA值显著升高,吞咽P/A比值明显降低;ADL评分显著改善,UPDRS第Ⅱ部分评分显著改善,第Ⅲ部分评分无显著变化。结论呼吸肌功能锻炼能够显著改善帕金森患者的咳嗽和吞咽功能,提高其生活质量。Objective To evaluate the effectiveness of respiratory muscle training on the activity of daily life in advanced Parkinson's disease patients with dysphagia.Methods A total of 37 patients were randomly assigned to control group(n=18)and treatment group(n=19).The control group received conventional treatment while the treatment group received respiratory muscle training for 30 days in addition to conventional treatment.Neurological deficit was assessed using IPD,CPD,EPRT,cough VA,P/A,ADL,and UPDRS.Results After 30 days of treatment,ICP,CPD,EPRT,cough VA,P/A,ADL and UPDRSA PartⅡscores were all better in the treatment group when compared with the control group(P<0.05).However,respiratory muscle training did not affect UPDRSA PartⅢ(P>0.05).Conclusion Respiratory muscle training is effective in improving the quality of life in advanced Parkinson's disease by improving the cough and swallow function.
