自体外周血造血干细胞移植和自体骨髓移植治疗急性白血病疗效的比较

摘　　要：目的比较自体外周血造血干细胞移植(APBSCT)或者净化自体骨髓移植(PABMT)对急性白血病的疗效。方法回顾性分析2020年8月至2022年8月河南科技大学第一附属医院收治的86例急性白血病患者的临床资料,根据治疗方式不同分成对照组(采取PABMT治疗)、观察组(采取APBSCT治疗),各43例。比较两组临床疗效、造血恢复时间、住院时间及移植并发症出现情况。结果两组治疗后1个月及3个月的缓解率比较,差异无统计学意义(P>0.05)。观察组的白细胞>1×10^(9)/L、中性粒细胞>0.5×10^(9)/L、血小板>20×10^(9)/L时间和住院时间均短于对照组,差异有统计学意义(P<0.05)。两组各类移植并发症出现情况比较,差异无统计学意义(P>0.05)。结论APBSCT、PABMT对急性白血病的疗效相当,但后者采集方便,能迅速恢复造血,缩短患者的住院时间,值得采用。Objective To compare the clinical efficacy of autologous peripheral blood stem cell transplantation(APBSCT)and primed autologous bone marrow transplantation(PABMT)in treating acute leukemia.Methods Clinical data of 86 patients with acute leukemia admitted to the First Affiliated Hospital of Henan University of Science and Technology from August 2020 to August 2022 were retrospectively analyzed.According to different treatment methods,they were divided into a control group(PABMT treatment)and an observation group(APBSCT treatment),with 43 cases in each group.The clinical efficacy,hematopoietic recovery time,hospital stay,and transplant complications were compared between the two groups.Results There was no significant difference in the remission rate of 1 month and 3 months after treatment between the two groups(P>0.05).The duration of Leukocytes>1×10^(9)/L,neutrophils>0.5×10^(9)/L,platelets>20×10^(9)/L and hospital stay in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the occurrence of various transplant complications between the two groups(P>0.05).Conclusion APBSCT and PABMT have the same effect on acute leukemia,but the latter is convenient to collect,can quickly restore hematopoiesis,and shorten the hospital stay of patients,so it is worth using.
