后腹腔镜联合电切镜治疗肾盂肿瘤的效果及对患者肾功能的影响

摘　　要：目的探讨后腹腔镜联合电切镜治疗肾盂肿瘤的效果及对患者肾功能的影响。方法选取2019年2月至2020年6月本院收治的80例肾盂肿瘤患者作为研究对象,按照随机数字表法分为对照组与实验组,每组40例。对照组行常规开放手术治疗,实验组行后腹腔镜联合电切镜治疗,比较两组围手术期相关指标、并发症发生率及肾功能指标。结果实验组手术时间、术后首次下床活动时间、胃肠功能恢复时间及住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。实验组并发症发生率为2.5%,明显低于对照组的20.0%,差异有统计学意义(P<0.05)。术后,实验组血清肌酐、尿素氮水平分别为(58.86±1.15)μmol/L、(2.94±1.04)mmol/L,均明显低于对照组的(158.47±2.21)μmol/L、(8.26±2.22)mmol/L,差异有统计学意义(P<0.05)。结论后腹腔镜联合电切镜治疗肾盂肿瘤疗效显著,可明显减轻患者手术创伤,缩短术后康复时间,减少手术并发症,改善肾功能。Objective To investigate the effect of retroperitoneal laparoscopy combined with electrosurgical resection in the treatment of renal pelvis tumors and its influence on renal function in patients.Methods 80 patients with renal pelvis tumors admitted to our hospital from February 2019 to June 2020 were selected as the study subjects,and they were divided into the control group and the experimental group according to random number table method,with 40 cases in each group.The control group was treated with routine open surgery,while the experimental group was treated with retroperitoneal laparoscopy combined with electrosurgical resection,the perioperative related indicators,incidence of complications,and renal function indicators were compared between the two groups.Results The surgical time,first postoperative ambulation time,gastrointestinal function recovery time,and hospitalization time in the experimental group were shorter than those in the control group,while the intraoperative bleeding was less than that in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the experimental group was 2.5%,which was significantly lower than the 20.0%in the control group,and the difference was statistically significant(P<0.05).After surgery,the serum creatinine and urea nitrogen levels in the experimental group were(58.86±1.15)μmol/L and(2.94±1.04)mmol/L,respectively,which were significantly lower than(158.47±2.21)μmol/L and(8.26±2.22)mmol/L in the control group,the differences were statistically significant(P<0.05).Conclusion Retroperitoneal laparoscopy combined with electrosurgical resection has significant efficacy in the treatment of renal pelvis tumors,which can significantly reduce surgical trauma,shorten postoperative recovery time,reduce surgical complications,and improve renal function.
