氯氮平联合盐酸帕罗西汀片治疗难治性精神分裂症的临床疗效

摘　　要：目的探究氯氮平联合盐酸帕罗西汀片治疗难治性精神分裂症的临床疗效。方法选取2019年12月至2020年12月本院收治的120例难治性精神分裂症患者作为研究对象,按照随机数字表法分为联合组与单一组,每组60例。单一组采用氯氮平治疗,联合组采用氯氮平联合盐酸帕罗西汀治疗,比较两组临床疗效、认知功能评分、述情障碍评分、简明精神病评定量表(BPRS)评分、阴性症状评定量表(SANS)评分、药物不良反应发生率。结果联合组治疗总有效率为98.33%,高于单一组的83.33%,差异有统计学意义(P<0.05)。治疗后,两组定向感、记忆力、注意力、口语理解、行为能力、语言评分均高于治疗前,且联合组高于单一组,差异有统计学意义(P<0.05)。治疗后,两组描述情感、认识区分、幻想、外向型思维评分均高于治疗前,且联合组高于单一组,差异有统计学意义(P<0.05)。治疗后,两组BPRS、SANS评分均低于治疗前,且联合组低于单一组,差异有统计学意义(P<0.05)。联合组用药不良反应率为1.67%,低于单一组的13.33%,差异有统计学意义(P<0.05)。结论氯氮平联合盐酸帕罗西汀联合治疗难治性精神分裂症临床疗效显著,可改善患者认知能力,降低述情障碍,药物应用安全性较高。Objective To explore the clinical efficacy of clozapine combined with paroxetine hydrochloride tablet in the treatment of refractory schizophrenia.Methods 120 patients with refractory schizophrenia admitted to our hospital from December 2019 to December 2020 were selected as the research subjects,they were divided into the combined group and the single group according to the random number table method,with 60 cases in each group.The single group was treated with clozapine,and the combined group was treated with clozapine combined with paroxetine hydrochloride,the clinical efficacy,cognitive function score,alexithymia score,brief psychiatric rating scale(BPRS)score,scale for assessment of negative symptoms(SANS)score,and incidence of adverse drug reaction were compared between the two groups.Results The total effective rate in the combined group was 98.33%,which was higher than 83.33%in the single group,and the difference was statistically significant(P<0.05).After treatment,the scores of sense of orientation,memory,attention,oral comprehension,behavioral ability,and language of the two groups were higher than those before treatment,and the combined group was higher than the single group,and the differences were statistically significant(P<0.05).After treatment,the scores of emotion description,cognitive differentiation,fantasy,and extroverted thinking of the two groups were higher than those before treatment,and the combined group was higher than the single group,and the differences were statistically significant(P<0.05).After treatment,the scores of BPRS and SANS of the two groups were lower than those before treatment,and the combined group were lower than the single group,and the differences were statistically significant(P<0.05).The adverse reaction rate in the combined group was 1.67%,which was significantly lower than 13.33%in the single group,and the difference was statistically significant(P<0.05).Conclusion Clozapine combined with paroxetine hydrochloride tablet in the treatment of refractory schizophren
