维A酸联合亚砷酸治疗急性早幼粒细胞白血病的临床价值

摘　　要：目的探讨维A酸联合亚砷酸治疗急性早幼粒细胞白血病(APL)的临床价值。方法选取2016年1月至2021年2月本院收治的78例APL患者作为研究对象,按照随机数字表法分为研究组与对照组,每组39例。两组均予以常规治疗,对照组给予维A酸治疗,研究组给予维A酸联合亚砷酸治疗,比较两组完全缓解率、症状缓解时间及不良反应发生率。结果两组完全缓解率比较差异无统计学意义。研究组完全缓解时间、凝血指标恢复正常时间、高白细胞血症持续时间均短于对照组,差异有统计学意义(P<0.05)。两组肝功能异常、皮肤问题、头痛、消化道反应、口干发生率比较差异无统计学意义。结论维A酸联合亚砷酸治疗APL,可缩短完全缓解时间、凝血指标恢复时间、高白细胞血症持续时间,且安全性较高,值得临床推广应用。Objective To investigate the clinical value of tretinoin combined with arsenous acid in the treatment of acute promyelocytic leukemia(APL).Methods 78 APL patients admitted to our hospital from January 2016 to February 2021 were selected as the research subjects,and they were divided into the study group and the control group according to the random number table method,with 39 cases in each group.Both groups were given conventional treatment,the control group was treated with tretinoin,and the study group was treated with tretinoin combined with arsenous acid,the complete response rate,symptom remission time and incidence of adverse reactions were compared between the two groups.Results There was no significant difference in complete response rate between the two groups.The complete remission time,coagulation index recovery time and duration of hyperleukocytosis in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of abnormal liver function,skin problems,headache,gastrointestinal reaction and dry mouth between the two groups.Conclusion Tretinoin combined with arsenous acid in the treatment of APL can shorten the complete remission time,coagulation index recovery time and duration of hyperleukocytosis,and has high safety,which is worthy of clinical promotion and application.
