牙周牙髓联合治疗第一磨牙牙周脓肿的临床疗效

摘　　要：目的探讨牙周牙髓联合治疗第一磨牙牙周脓肿的临床疗效。方法选取2019年8月至2020年8月本院收治的79例第一磨牙牙周脓肿患者作为研究对象,按照随机数字表法分为观察组(n=39)与对照组(n=40)。对照组采用牙周治疗,观察组在对照组基础上联合牙髓治疗,比较两组临床疗效及治疗前后牙周牙髓相关指标、炎症因子表达水平。结果观察组治疗总有效率为94.87%,高于对照组的77.50%,差异有统计学意义(P<0.05)。观察组菌斑指数、龈沟出血指数均低于对照组,牙齿松动度小于对照组,牙周袋探诊深度深于对照组,差异有统计学意义(P<0.05)。治疗前,两组白细胞介素(IL)-6、IL-1β水平比较差异无统计学意义;治疗后,两组IL-6、IL-1β水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论牙髓联合牙周治疗在提高牙周脓肿疗效确切,可改善患者牙髓牙周指标及局部炎症指标,值得临床推广应用。Objective To investigate the clinical effect of periodontal pulp combined treatment on periodontal abscess of first molar.Methods 79 patients with periodontal abscess of first molar admitted to our hospital from August 2019 toAugust 2020 were selected as the study subjects,and they were divided into the observation group(n=39)and the control group(n=40)according to random number table method.The control group received periodontal treatment,and the observation group was combined with pulp treatment on the basis of the control group,the clinical efficacy and periodontal endodontic related indexes and the expression level of inflammatory factors before and after treatment were compared between the two groups.Results The total effective rate in the observation group was 94.87%,which was higher than 77.50%in the control group,the difference was statistically significant(P<0.05).The plaque index and gingival sulcus bleeding index in the observation group were lower than those in the control group,the tooth looseness was lower than that in the control group,and the depth of periodontal pocket exploration was deeper than that in the control group,the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in interleukin(IL)-6 and IL-1βlevels between the two groups;after treatment,IL-6 and IL-1βlevels in two groups were lower than before treatment,and observation group was lower than control group,the difference was statistically significant(P<0.05).Conclusion Endodontic combined periodontal therapy is effective in improving periodontal abscess,and can improve endodontic periodontal indexes and local inflammatory state of patients,which is worthy of clinical promotion and application.
