腹腔镜辅助阴式全子宫切除术对巨大子宫肌瘤患者激素水平及性功能的影响

摘　　要：目的探讨腹腔镜辅助阴式全子宫切除术(LAVH)对巨大子宫肌瘤患者激素水平及性功能的影响。方法选取2019年7月至2021年7月山东国欣颐养集团新汶中心医院收治的74例巨大子宫肌瘤患者作为研究对象,按照随机数字表法分为对照组与研究组,每组37例。对照组行传统开腹子宫切除术(TAH)治疗,研究组行LAVH治疗,比较两组激素[雌激素受体(ER)、孕激素受体(PR)]水平、性功能、术后恢复情况(肛门排气时间、下床活动时间、住院时间、疼痛程度)及并发症发生率。结果术前,两组ER及PR阳性细胞百分比比较差异无统计学意义;术后48 h,两组ER及PR阳性细胞百分比均低于术前,且研究组低于对照组,差异有统计学意义(P<0.05)。术前,两组性欲、性唤起、阴道润滑度、性高潮、性生活满意度、性交疼痛评分及总分比较差异无统计学意义;术后,两组性欲、性唤起、性生活满意度、性交疼痛评分及总分均低于术前,对照组阴道润滑度、性高潮评分均低于术前,研究组性欲、性唤起、阴道润滑度、性高潮、性生活满意度、性交疼痛评分及总分均高于对照组,差异有统计学意义(P<0.05);研究组手术前后阴道润滑度、性高潮评分比较差异无统计学意义。研究组肛门排气时间、下床活动时间、住院时间均短于对照组,视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率为5.41%,低于对照组的21.62%,差异有统计学意义(P<0.05)。结论LAVH治疗巨大子宫肌瘤患者效果良好,可促进激素水平恢复正常,对性功能影响较小,并发症发生风险较低,值得临床推广应用。Objective To investigate the effect of laparoscopic assisted vaginal hysterectomy(LAVH)on hormone level and sexual function in patients with giant hysteromyoma.Methods 74 patients with giant hysteromyoma admitted to Xinwen Central Hospital of Shandong Guoxin Yiyang Group from July 2019 to July 2021 were selected as the research subjects,and they were divided into the control group and the study group according to random number table method,with 37 cases in each group.The control group was treated with traditional abdominal hysterectomy(TAH),and the study group was treated with LAVH,the hormone(estrogen receptor[ER],progesterone receptor[PR])levels,sexual function,postoperative recovery(anal exhaust time,ambulation time,hospitalization time,pain degree)and the incidence of complications were compared between the two groups.Results Before operation,there were no significant differences in the percentage of ER and PR positive cells between the two groups;at 48 h after operation,the percentage of ER and PR positive cells of the two groups was lower than before operation,and the study group was lower than the control group,the differences were statistically significant(P<0.05).Before operation,there were no significant differences in sexual desire,sexual arousal,vaginal lubrication,orgasm,sexual satisfaction,sexual pain scores and total score between the two groups;after operation,the scores of sexual desire,sexual arousal,sexual satisfaction,pain during intercourse and total score of the two groups were lower than before operation;the scores of vaginal lubrication and orgasm in the control group were lower than before operation;the scores of sexual desire,sexual arousal,vaginal lubrication,orgasm,sexual satisfaction,pain during intercourse and total score in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05),there were no significant differences in vaginal lubrication and orgasmic score before and after operation.The anal exhaust time,ambulation time
