摘　　要：目的探讨术中细胞学涂片与冷冻切片在甲状腺乳头状癌(PTC)快速诊断中的价值。方法选取2020年8月至2021年8月本院收治的86例疑似PTC患者作为研究对象。所有患者均行冷冻切片、细胞学涂片检查,以病理检查结果为金标准,比较单纯冷冻切片、冷冻切片联合细胞学涂片检查的检出情况、诊断效能。结果86例疑似PTC患者中,病理检查确诊80例;冷冻切片联合细胞学涂片检查检出率高于单纯冷冻切片检查,差异有统计学意义(P<0.05)。冷冻切片联合细胞学涂片检查的灵敏度、准确度均高于单纯冷冻切片,差异有统计学意义(P<0.05);两种不同检查方法的特异度、阳性预测值、阴性预测值比较差异无统计学意义。Kappa检验结果显示,冷冻切片联合细胞学涂片检查与病理检查结果的一致性较为理想(P<0.05);单纯冷冻切片检查与病理检查结果的一致性较差。结论冷冻切片联合细胞学涂片检查在PTC患者诊断中效果确切,灵敏度、准确度高,可减少漏诊、误诊的发生,与病理结果一致性较为理想,可为PTC快速诊断及手术的顺利开展提供参考依据。Objective To explore the value of intraoperative cytology smear and frozen sections in the rapid diagnosis of papillary thyroid carcinoma(PTC).Methods 86 suspected PTC patients admitted to our hospital from August 2020 to August 2021 were selected as the study subjects.All patients underwent frozen section and cytology smear examination,the results of pathological examination were used as the gold standard,the detection and diagnostic efficiency of simple frozen section and frozen section combined with cytology smear examination were compared.Results Among 86 suspected PTC patients,80 cases were confirmed by pathological examination;the detection rate of frozen section combined with cytology smear was higher than that of frozen section alone,and the difference was statistically significant(P<0.05).The sensitivity and accuracy of frozen section combined with cytology smear were higher than those of frozen section alone,and the differences were statistically significant(P<0.05);there were no significant differences of specificity,positive predictive value and negative predictive value between the two methods.The results of Kappa test showed that the results of frozen section combined with cytology smear were in good agreement with those of pathological examination(P<0.05);the consistency of frozen section examination and pathological examination was poor.Conclusion Frozen sectioncombined with cytology smear examination has precise effect,high sensitivity and accuracy in the diagnosis of PTC patients,which is conducive to reducing the occurrence of missed diagnosis and misdiagnosis,and is in good agreement with pathological results,which can provide reference for the rapid diagnosis of PTC and the smooth development of surgical treatment.
