阴道镜检查联合液基薄层细胞学检查在妇科门诊宫颈癌高危人群中的筛查价值

摘　　要：目的探究阴道镜检查联合液基薄层细胞学检查(TCT)在妇科门诊宫颈癌高危人群中的筛查价值。方法选取2020年1—12月本院收治的156例宫颈癌高危患者作为研究对象,患者均分别进行阴道镜检查、TCT。以病理组织检查结果为金标准,比较两种诊断方式单独及联合诊断的价值。结果经病理组织诊断,156例宫颈癌高危患者中,阴性137例,阳性19例。阴道镜检查、TCT单独及联合诊断准确度、灵敏度、特异度比较差异有统计学意义(P<0.05),其中联合诊断准确度、特异度均高于阴道镜检查、TCT,灵敏度高于TCT,阴道镜检查灵敏度高于TCT,特异度低于TCT,差异有统计学意义(P<0.05),其他检查方法准确度、灵敏度、特异度两两比较差异无统计学意义。结论阴道镜检查联合液基薄层细胞学检查可提高妇科门诊宫腔镜高危人群宫颈癌筛查的诊断准确度、灵敏度,提升诊断质量。Objective To explore the clinical value of colposcopy combined with thinprep cytologic test(TCT)in cervical cancer screening for high-risk population of cervical cancer in gynecological clinic.Methods 156 cases of high-risk patients with cervical cancer in our hospital from January to December 2020 were selected as the research subjects,colposcopy and TCT were performed respectively,the results of pathological examination were taken as the gold standard to evaluate the diagnostic value of the two diagnostic methods.Results Among 156 high-risk patients with cervical cancer,137 were negative and 19 were positive.There were significant differences in the accuracy,sensitivity and specificity of colposcopy examination,TCT alone and combined diagnosis(P<0.05).The accuracy and specificity of combined diagnosis were higher than those of colposcopy examination and TCT,and the sensitivity was higher than that of TCT,the sensitivity of colposcopy examination was higher than that of TCT,and the specificity was lower than that of TCT,the differences were statistically significant(P<0.05),there were no significant differences in the accuracy,sensitivity and specificity of other examination methods.Conclusion The application of colposcopy combined with TCT in the cervical cancer screening in the high-risk population of hysteroscopy in gynecological clinic can improve the accuracy,sensitivity and quality of diagnosis.
