摘  要:目的:基于图论探讨血管性认知功能障碍(Vascular cognitive impairment,VCI)半球脑网络拓扑属性的异常。材料与方法:前瞻性地对61名VCI患者及38名健康对照组进行静息态fMRI扫描,利用基于MATLAB的GRETNA构建45×45的半球脑网络,并采用双样本t检验比较组间半球网络全局及局部网络属性的差异(Bonferroni验证,P<0.05)。结果:与HC组相比,VCI患者右侧大脑半球表现出全局网络拓扑结构的破坏,右侧大脑半球标准化特征路径长度λ、特征路径长度L强度增加,全局网络效率(Networkefficency,Eglob)减低。VCI半球脑网络局部属性亦出现不同程度的异常,左侧大脑半球眶内额上回介度中心度、眶内额上回节点效率、枕下回节点效率、顶上回节点局部效率、右侧大脑半球海马旁回、缘上回、豆状苍白球的节点效率降低;左侧大脑半球的海马介度中心度、距状裂周围皮层、舌回及楔前叶的节点度及中央后回的节点局部效率增强,这可能是对脑功能网络效率降低的一种代偿,也反映了脑部神经纤维的可塑性。结论:本实验结果表明,VCI患者表现出半球脑功能网络的拓扑组织失调,这对理解VCI的病理生理机制有重要意义。为VCI的早期诊断、病程监测、动态观察及预后评估提供了可靠的生物学标记。Purpose:To Explore the abnormalities of topological properties of hemispheric brain networks of vascular cognitive dysfunction based on the graph theory.Materials and Methods:Prospectively,61 VCI patients and 38 healthy controls were scanned with resting-fMRI,and a 45×45 hemispheric brain network was constructed using MATLAB-based GRETNA,and the dfferences in global and local network properties of hemispheric networks between groups were compared.Results:Compared with the HC group,VCI patients in the right cerebral hemisphere exhibited disruption of global network topology,increased intensity ofλand Lp in the right cerebral hemisphere,and reduced Eglob.Local properties of the VCI hemispheric brain network also showed varying degrees of abnormality.The Bc and Ne of ORBsupmed.L、Ne of IOG.L、NLe of SPG.L、Ne of PHG.R、Ne of SMG.R and Ne of PAL.R decreased.The Bc of HIP.L、Dc of CAL.L、LING.L、PCUN.L and Nle of PoCG.L increased.It may be a compensation for the reduced eficiency of functional brain networks and a response to the plasticity of nerve fibers in the brain.Conclusion:The results of this experiment showed that patients with VCI exhibit topographical disorganization of the hemispheric brain functional network,which is important for understanding the pathophysiological mechanisms of VCI.It provides a reliable biological marker for early diagnosis,disease course monitoring,dynamic observation and prognostic assessment of VCI.
