摘 要:目的探讨老年阿尔茨海默病患者血清维生素D、INF-γ、IL-4、IL-33水平变化、相关性及危险因素。方法选取我院2019年8月至2021年8月期间首次确诊并入院治疗的AD患者104例。根据受试者首次发病年龄分为老年AD组及非老年AD组,其中老年AD组71例,非老年AD组33例。比较两组血清维生素D水平、血清INF-γ、IL-4、IL-33水平;分析血清维生素D水平与INF-γ、IL-4、IL-33等指标的相关性;将老年AD单因素分析中有统计学意义的项目带入多因素Logistic回归分析,得出老年阿尔茨海默病独立危险因素。结果老年AD组患者血清维生素D水平明显低于非老年AD组(P<0.05);老年AD组患者血清INF-γ、IL-4、IL-33水平明显高于非老年AD组(P<0.05);年龄、TC、LDL-C、空腹血糖、Scr、INF-γ、IL-4及IL-33与血清维生素D水平呈负相关(P<0.05);将年龄、受教育程度、高血压病史、糖尿病史、总胆固醇水平、低密度脂蛋白水平、随机血糖、血清维生素D水平、INF-γ、IL-4及IL-33等11项危险因素带入多因素Logistic分析得出,年龄、受教育程度、高血压病史、随机血糖水平、血清维生素D、INF-γ、IL-4及IL-33是老年阿尔茨海默病的独立危险因素(P<0.05)。结论老年AD患者血清维生素D明显降低,INF-γ、IL-4、IL-33水平明显升高,血清维生素D水平与INF-γ、IL-4、IL-33呈负相关,血清维生素D、INF-γ、IL-4及IL-33对AD的临床诊疗发作重要的作用。Objective To investigate serum vitamin D and INF-γin elderly patients with Alzheimer’s disease for their changes,correlation and related risk factors of IL-4 and IL-33 levels.Methods 104 AD patients first diagnosed and hospitalized in our hospital from August,2019 to August,2021 were selected.According to the first onset age,these subjects were divided into the elderly AD group and non-elderly AD group,including 71 cases in the elderly AD group and 33 cases in the non-elderly AD group.Levels of serum vitamin D and serum INF-γwere compared between the two groups,as well as IL-4 and IL-33 levels;The relationship between serum vitamin D level and inf was analyzed.The correlations of IL-4,IL-33 and other indicators were evaluated;The statistically significant items in the univariate analysis of elderly AD were included into multivariate logistic regression analysis to obtain the independent risk factors of elderly Alzheimer’s disease.Results The level of serum vitamin D in the elderly AD group was significantly lower than that in the non-elderly AD group(P<0.05);Serum INF-γin the elderly AD group,as well as levels of IL-4 and IL-33 were significantly higher than those in the non-elderly AD group(P<0.05);Age,TC,LDL-C,fasting blood glucose,SCR,INF-γ,IL-4 and IL-33 were negatively correlated with serum vitamin D levels(<0.05);Age,education level,history of hypertension,history of diabetes,total cholesterol,low density lipoprotein,random blood sugar,serum vitamin D level,INF-γ11 risk factors such as IL-4 and IL-33 were included into multivariate logistic analysis.The results were as following:age,education,history of hypertension,random blood glucose level,serum vitamin D and INF-γ,IL-4 and IL-33 were independent risk factors for senile Alzheimer’s disease(P<0.05).Conclusion Serum vitamin D in elderly patients with AD decreases significantly,INF-γ,levels of IL-4 and IL-33 increase significantly,and the level of serum vitamin D is correlated with INF-γ.IL-4 and IL-33 are negatively correlated with serum vita
