铸瓷嵌体在后牙缺损修复中的效果及对龈沟液炎症因子和基质金属蛋白酶-8水平的影响

摘　　要：目的探讨铸瓷嵌体在后牙缺损修复中的效果及对龈沟液炎症因子和基质金属蛋白酶-8(MMP-8)水平的影响。方法选取2018年3月至2019年5月本院口腔科收治的100例(124颗牙)修复后牙体缺损患者作为研究对象,按照随机数字表法分为对照组(n=50,58颗牙)与研究组(n=50,66颗牙)。对照组行聚合瓷嵌体修复,研究组行铸瓷嵌体修复,比较两组修复后不同时期缺损修复效果、修复前后龈沟液指标[白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、MMP-8]及满意度。结果修复后6个月,两组缺损修复效果比较差异无统计学意义;修复后12个月,研究组缺损修复效果优于对照组,差异有统计学意义(P<0.05)。修复后6个月,对照组龈沟液IL-8、TNF-α、MMP-8水平均高于修复前,差异有统计学意义(P<0.05);研究组修复术前后龈沟液IL-8、TNF-α、MMP-8水平比较差异无统计学意义;修复后6个月,研究组龈沟液IL-8、TNF-α、MMP-8水平均明显低于对照组,差异有统计学意义(P<0.05)。研究组总满意度为98.00%,高于对照组的84.00%,差异有统计学意义(P<0.05)。结论铸瓷嵌体修复后牙缺损效果显著,可降低龈沟液IL-8、TNF-α、MMP-8水平,减轻对牙周组织的刺激,生物相容性良好,还能提高患者满意度,值得临床推广应用。Objective To investigate the effect of cast porcelain inlay in the restoration of posterior teeth defect and its influence the levels of in-flammatory factors,matrix metalloproteinase-8(MMP-8)in gingival crevicular fluid.Methods 100 patients(124 teeth)with dental defects after res-toration admitted to the department of Stomatology of our hospital from March 2018 to May 2019 were selected as the study subjects,and they were divided into the control group(n=50,58 teeth)and the study group(n=50,66 teeth)according to the random number table method.The control group was repaired with polymeric porcelain inlay,and the study group was repaired with cast porcelain inlay,the repair effect at different periods after res-toration,gingival crevicular fluid index(interleukin-8[IL-8],tumor necrosis factor-α[TNF-α],MMP-8)before and after repair and satisfaction were compared between the two groups.Results At 6 months after repair,there was no significant difference in the repair effect between the two groups;at 12 months after the repair,the repair effect in the study group was better than that in the control group,and the difference was statistically signifi-cant(P<0.05).At 6 months after repair,the levels of IL-8,TNF-αand MMP-8 in gingival crevicular fluid in the control group were higher than those before repair,and the differences were statistically significant(P<0.05);there was no significant difference in the levels of IL-8,TNF-αand MMP-8 in gingival crevicular fluid before and after repair in the study group;6 months after repair,the levels of IL-8 and TNF-αand MMP-8 in gin-gival crevicular fluid in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The total satisfaction rate in the study group was 98.00%,which was higher than 84.00%in the control group,the difference was statistically significant(P<0.05).Conclusion The cast porcelain inlay has significant effect on the restoration of posterior tooth defects,which can reduce the levels
