云南白药胃管注入联合奥美拉唑治疗新生儿上消化道出血的临床疗效及对炎症因子水平的影响

摘　　要：目的探讨云南白药胃管注入联合奥美拉唑治疗新生儿上消化道出血的临床疗效及对炎症因子水平的影响。方法选取2018年3月至2021年2月本院收治的76例上消化道出血新生儿作为研究对象,按照随机数字表法分为观察组与对照组,每组38例。对照组给予奥美拉唑治疗,观察组在对照组基础上联合云南白药治疗,比较两组临床疗效、症状缓解时间及炎症因子水平。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组止血时间、大便转黄时间、腹痛缓解时间、大便隐血试验阴性时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,两组白细胞介素-2(IL-2)水平均高于治疗前,白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均低于治疗前,且观察组IL-2水平高于对照组,IL-6、TNF-α水平均明显低于对照组(P<0.05)。结论云南白药胃管注入联合奥美拉唑治疗新生儿上消化道出血效果显著,能明显缩短症状改善时间,调节炎症因子水平,值得临床推广应用。Objective To investigate clinical effect of Yunnan Baiyao gastric tube injection combined with omeprazole in the treatment of neonatal upper gastrointestinal bleeding and its effect on levels of inflammatory factors.Methods 76 cases of neonatal upper gastrointestinal bleeding admitted to our hospital from March 2018 to February 2021 were selected as the study subjects,and they were divided into the observation group and the control group according to the random number table method,with 38 cases in each group.The control group given omeprazole treatment,the observation group was combined with Yunnan Baiyao treatment on the basis of the control group,the clinical efficacy,symptom relief time,inflammatory factors level were compared between the two groups.Results The total treatment efficiency in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).The hemostasis time,stool turning time,abdominal pain relief time,stool occult blood test negative time in observation group were shorter than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-2(IL-2)of the two groups were higher than before treatment,and the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)of the two groups were lower than before treatment,and the level of IL-2 in the observation group was higher than that in the control group,and the levels of IL-6 and TNF-αwere significantly lower than those in the control group(P<0.05).Conclusion Yunnan Baiyao gastric tube injection combined with omeprazole has significant effect in the treatment of neonatal upper gastrointestinal bleeding,which can significantly shorten the improvement time of symptoms,and regulate the level of inflammatory factors,and is worthy of clinical promotion and application.
