摘　　要：目的探讨鼻内镜下鼻腔泪囊吻合术(DCR)治疗泪道阻塞性疾病的临床效果。方法选取2019年8月至2021年8月本院收治的100例泪道阻塞性疾病患者作为研究对象,采用随机数字表法分为对照组与研究组,每组50例。对照组采用传统外路DCR治疗,研究组采用鼻内镜下DCR治疗,比较两组治疗后手术指标、临床疗效、术后并发症发生情况、生命质量、复发情况。结果研究组治疗总有效率显著高于对照组(P<0.05);研究组术中出血量、VAS评分及术后复发率均低于对照组,住院时间短于对照组,差异有统计学意义(P<0.05);研究组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);术后,两组心理功能、社会功能、生理功能、精神状态评分均高于术前,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组复发率低于对照组,差异有统计学意义(P<0.05)。结论鼻内镜下鼻腔泪囊吻合术治疗泪道阻塞性疾病疗效显著,具有术中出血量少、疼痛程度轻、康复速度快、复发率、并发症发生率低等优势,可改善患者生命质量,值得临床推广应用。Objective To investigate the clinical effect of endoscopic dacryocystorhinostomy(DCR)in the treatment of lacrimal duct obstructive diseases.Methods 100 patients with lacrimal duct obstructive diseases admitted to our hospital from August 2019 to August 2021 were selected as research subjects,and they were divided into control group and study group according to random number table method,with 50 cases in each group.The control group was treated with traditional external DCR,while the study group was treated with DCR under nasal endoscope.The surgical indicators,treatment effects,postoperative complications,quality of life,and recurrence rate were compared between the two groups.Results The total effective rate of treatment in the study group was significantly higher than that in the control group(P<0.05);The amount of intraoperative bleeding,visual analogue scale(VAS)score,and postoperative recurrence rate in the study group were lower than those in the control group,and the hospital stay was shorter than that in the control group,the differences were statistically significant(P<0.05);the incidence of postoperative complication in the study group was significantly lower than that in the control group,the difference was statistically significant(P<0.05);after surgery,the scores of psychological function,social function,physiological function,and mental state in the two groups were higher than those before surgery,and the study group was higher than the control group,the differences were statistically significant(P<0.05).The recurrence rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Nasal endoscopic dacryocystorhinostomy has significant efficacy in the treatment of lacrimal duct obstructive diseases,with advantages such as less intraoperative bleeding,less pain,rapid recovery,low recurrence rate,and complications,it can improve the quality of life of patients,and is worthy of clinical promotion and application.
