超声生物显微镜下前房结构参数与原发性闭角型青光眼合并白内障患者最佳矫正视力的相关性分析

摘  要:目的探讨超声生物显微镜(UBM)下前房结构参数与原发性闭角型青光眼(PACG)合并白内障患者最佳矫正视力(BCVA)的相关性。方法选取2019年1月至2021年3月于本院确诊的80例白内障患者作为研究对象,根据入院检查结果分为PACG组(n=30)和无PACG组(n=50),使用天津迈达MD-300LUBM检查两组中央前房深度(ACD)、房角开放距离500(AOD_(500))、小梁睫状体距离(TCPD)、小梁虹膜夹角(TIA)、虹膜厚度(IT)、虹膜悬韧带距离(IZD)、虹膜晶状体夹角、巩膜睫状体夹角,采用非接触式眼压计检测两组眼压(IOP),采用糖尿病视网膜病变早期治疗研究(ETDRS)视力表检查两组BCVA水平。结果PACG组ACD浅于无PACG组,AOD_(500)、TCPD、TIA、IZD、虹膜晶状体夹角、巩膜睫状体夹角均小于无PACG组,IT(IT1、IT2、IT3)薄于无PACG组,差异有统计学意义(P<0.05)。PACG组IOP明显高于无PACG组,BCVA明显低于无PACG组,差异有统计学意义(P<0.05);Pearson相关分析结果显示,ACD、AOD_(500)、TCPD、TIA、IT、IZD、虹膜晶状体夹角、巩膜睫状体夹角与BCVA呈正相关(r>0,P<0.05)。结论UBM下前房结构参数与PACG合并白内障患者的BCVA呈正相关,临床可根据前房结构参数的变化评估患者的视力状况。Objective To explore the correlation between anterior chamber structure parameters and best corrected visual acuity(BCVA)in patients with primary angle-closed glaucoma(PACG)and cataract under ultrasonic biological microscope(UBM).Methods A total of 80 patients with cataract confirmed in our hospital from January 2019 to March 2021 were enrolled as the research subjects,they were divided into PACG group(n=30)and non-PACG group(n=50)according to examination results after admission.The anterior chamber depth(ACD),angle opening distance 500(AOD_(500)),meshwork ciliary process distance(TCPD),trabecular iris angle(TIA),iris thickness(IT),distance of iris suspensory ligament(IZD),irislens angle and sclera-ciliary body angle in both groups were examined by Tianjin Maida MD-300L UBM,the intraocular pressure(IOP)was measured by non-contact tonometer,BCVA was detected by early treatment diabetic retinopathy study(ETDRS)eye chart.Results The ACD of the PACG group was shallower than that of the non-PACG group,the AOD_(500),TCPD,TIA,IZD,iris-lens angle and sclera-ciliary body angle were all smaller than those of the non-PACG group,IT(IT1,IT2,IT3)was thinner than that of the non-PACG group,and the differences were statistically significant(P<0.05).IOP in PACG group was significantly higher than that in non-PACG group,while BCVA was significantly lower than that in non-PACG group,the difference was statistically significant(P<0.05).The results of Pearson correlation analysis showed that ACD,AOD_(500),TCPD,TIA,IT,IZD,iris-lens angle and sclera-ciliary body angle were positively correlated with BCVA(P<0.05).Conclusion The anterior chamber struc-ture parameters are positively correlated with BCVA in patients with PACG and cataract under UBM,Clinically,visual acuity of patients can be evaluated according to the changes of anterior chamber structure parameters.
