参芪仙补汤防治急性髓细胞白血病患者因柔红霉素所致的亚临床心肌损害的疗效及对B型利钠肽和P-R间期的影响

摘　　要：目的探究参芪仙补汤防治急性髓细胞白血病(AML)患者因柔红霉素导致的亚临床心肌损害的临床疗效及对B型利钠肽(BNP)和P-R间期的影响。方法选取2017年5月至2019年6月本院收治的70例AML患者作为研究对象,按照随机数字表法分为两组,各35例。对照组给予化疗干预,研究组在对照组基础上联合参芪仙补汤治疗,比较两组临床疗效、心脏毒性反应分级及治疗前后BNP水平与P-R间期。结果研究组治疗总有效率为82.86%,高于对照组的60.00%,差异有统计学意义(P<0.05)。研究组心脏毒性反应分级情况优于对照组,差异有统计学意义(P<0.05)。治疗前,两组BNP水平及P-R期间比较差异无统计学意义;治疗后,研究组BNP水平低于对照组,P-R期间短于对照组,差异有统计学意义(P<0.05)。结论参芪仙补汤防治AML患者因柔红霉素导致的亚临床心肌损害疗效确切,能有效稳定病情,降低心脏毒性反应分级,抑制BNP水平上升及P-R期间增大,对保护心脏具有重要作用,值得临床推广应用。Objective To investigate the clinical efficacy of Shenqi Xianbu decoction on subclinical myocardial damage caused by daunorubicin in patients with acute myeloid leukemia(AML)and its effect on brain natriuretic peptide(BNP)and P-R interval.Methods 70 AML patients admitted to our hospital from May 2017 to June 2019 were selected as the research subjects,and they were divided into two groups according to the random number table,with 35 cases in each group.The control group was given chemotherapy intervention,and the study group was treated with Shenqi Xianbu decoction on the basis of the control group,the clinical efficacy,cardiac toxicity grade and BNP level and P-R interval before and after treatment were compared between the two groups.Results The total effective rate of the study group was 82.86%,which was higher than 60.00%of the control group,the difference was statistically significant(P<0.05).The grading of cardiac toxicity in the study group was better than that in the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in BNP level and P-R period between the two groups;after treatment,the BNP level in the study group was lower than that in the control group,and the P-R period was shorter than that in the control group,the differences were statistically significant(P<0.05).Conclusion Shenqi Xianbu decoction has definite effect on the prevention and treatment of subclinical myocardial damage caused by daunorubicin in AML patients,it can effectively stabilize the condition,reduce the grade of cardiac toxic reaction,inhibit the increase of BNP level and increase during P-R,and has an important role in protecting the heart,which is worthy of clinical application.
