天丹通络胶囊联合替格瑞洛对老年脑梗死患者神经细胞因子和血小板反应蛋白-1及血管内皮生长因子水平的影响

摘　　要：目的探讨天丹通络胶囊联合替格瑞洛对老年脑梗死患者神经细胞因子、血小板反应蛋白-1(TSP-1)、血管内皮生长因子(VEGF)水平的影响。方法选取2020年7月至2021年7月本院收治的80例脑梗死老年患者作为研究对象,按照随机数字表法分为两组,每组40例。对照组给予替格瑞洛治疗,观察组给予天丹通络胶囊联合替格瑞洛治疗,比较两组治疗前后神经功能和神经细胞因子、TSP-1、VEGF水平及不良反应发生情况。结果治疗前,两组美国国立卫生研究院卒中量表(NIHSS)评分比较差异无统计学意义;治疗后,两组NIHSS评分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组血清神经元特异性烯醇化酶(NSE)、神经生长因子(NGF)、神经营养因子(NTF)水平比较差异无统计学意义;治疗后,两组血清NSE水平均低于治疗前,血清NGF、NTF水平均高于治疗前,且观察组血清NSE水平低于对照组,血清NGF、NTF水平均高于对照组,差异有统计学意义(P<0.05)。治疗前,两组血清VEGF、TSP-1水平比较差异无统计学意义;治疗后,两组TSP-1水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,观察组VEGF水平高于治疗前及对照组,差异有统计学意义(P<0.05),但对照组治疗前后VEGF水平比较差异无统计学意义。两组不良反应发生率比较差异无统计学意义。结论天丹通络胶囊联合替格瑞洛治疗老年脑梗死患者效果确切,可显著改善患者神经功能,有效调节患者神经细胞因子、TSP-1、VEGF水平,值得临床推广应用。Objective To analyze the effect of Tiandan Tongluo capsule combined with ticagrelor on the levels of neurocytokine and thrombos-pondin-1(TSP-1)and vascular endothelial growth factor(VEGF)in elderly patients with cerebral infarction.Methods 80 elderly patients with cere-bral infarction admitted to our hospital from July 2020 to July 2021 were selected as the study subjects,they were divided into two groups according to the random number table method,with 40 cases in each group.The control group was treated with ticagrelor,and the observation group was treat-ed with Tiandan Tongluo capsule combined with ticagrelor,the neural function,the level of neurocytokines,TSP-1,VEGF before and after treatment,and adverse reactions were compared between the two groups.Results Before treatment,there was no significant difference in National Institutes of Health stroke scale(NIHSS)scores between the two groups;after treatment,the NIHSS score of the two groups was lower than before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Before treatment,there was no signifi-cant difference in the levels of serum neuron specific enolase(NSE),nerve growth factor(NGF),neurotrophic factor(NTF)between the two groups;after treatment,the serum levels of NSE of the two groups were lower than before treatment,the serum levels of NGF,NTF were higher than before treatment,and the serum level of NSE in the observation group was lower than that in the control group,and the serum levels of NGF and NTF were higher than those in the control group,the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in serum levels of VEGF and TSP-1 between the two groups;after treatment,the level of TSP-1 of the two groups were lower than that before treat-ment,and the observation group was lower than the control group,the differences were statistically significant(P<0.05);after treatment,the level of VEGF in observation group was higher than
