摘　　要：目的探讨内镜黏膜下剥离术对早期胃癌患者血清白细胞介素水平及T细胞亚群变化的影响。方法选取2017年9月至2020年7月于本院行内镜黏膜下剥离术治疗的30例早期胃癌患者作为观察组,另选取同期于本院行胃癌根治术治疗的30例早期胃癌患者作为对照组。比较两组血清白细胞介素-17(IL-17)、白细胞介素-23(IL-23)水平和T细胞亚群变化。结果术前,两组血清IL-17、IL-23水平比较差异无统计学意义;术后,两组血清IL-23、IL-17水平均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。术前,两组CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平比较差异无统计学意义;术后,两组CD4^(+)、CD4^(+)/CD8^(+)水平均低于术前,CD8^(+)水平均高于术前,但观察组CD4^(+)、CD4^(+)/CD8^(+)水平高于对照组,CD8^(+)低于对照组低,差异有统计学意义(P<0.05)。结论内镜黏膜下剥离术治疗早期胃癌患者的效果较好,可降低患者血清白细胞介素水平,调节其免疫功能,利于病情好转。Objective To investigate the effect of endoscopic submucosal dissection on serum interleukin level and T cell subsets in patients with early gastric cancer.Methods 30 patients with early gastric cancer who underwent endoscopic submucosal dissection in our hospital from Septem-ber 2017 to July 2020 were selected as the observation group,and another 30 patients with early gastric cancer who underwent radical gastrectomy in our hospital during the same period were selected as the control group.The serum levels of interleukin-17(IL-17),interleukin-23(IL-23)and the changes of T cell subsets were compared between the two groups.Results Before operation,there was no significant difference in the levels of IL-17 and IL-23 between the two groups;after operation,the serum levels of IL-23 and IL-17 in the two groups were lower than those before operation,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Before operation,there was no sta-tistically significant difference in the levels of CD4^(+),CD8^(+),CD4^(+)/CD8^(+)between the two groups;after operation,the levels of CD4^(+),CD4^(+)/CD8^(+)in the two groups were lower than those before operation,and the level of CD8^(+)was higher than that before operation,but the levels of CD4^(+),CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and the level of CD8^(+)was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Endoscopic submucosal dissection is effective in the treatment of patients with early gastric cancer,can reduce the level of serum interleukin and regulate the immune function of patients,and help to improve the condition.
