基于数据挖掘探讨“卢火神”治疗子宫内膜癌遣方用药规律

摘　　要：分析总结卢铸之治疗子宫内膜癌的处方用药特点,以期为今后临床治疗子宫内膜癌提供参考。收集《卢氏临证实验录》中卢铸之治疗子宫内膜癌的处方,使用Excel建立数据库,运用IBM SPSS Modeler 18.0的关联规则分析常用药物组合,采用Cytoscape 3.8.0、SPSS Statistics 22进行数据挖掘分析。处方共涉及54种药,使用515次。高频中药包括炙甘草、制附片、砂仁、白术和当归等;药性以温、热为主,药味以辛、甘为主,归经主要涉及脾、肝、肾三脏;药物用量跨度大,大剂量药物多。核心药物组为制附片-白术-砂仁-当归-煨姜-炙甘草;聚类分析得出6组聚类组;主成分分析共提取11个公因子;以制附片为因变量的决策树体现出苍术、蒲黄、煨姜、制升麻在配伍过程中的重要地位。利用数据挖掘技术可有效分析出卢铸之治疗子宫内膜癌的处方用药规律。该病的形成以阳虚内寒为本,瘀血疼痛为标,应时刻注意固中宫,调升降,以扶阳抑阴为体,气化消瘀为用。In order to provide reference for clinical treatment of endometrial cancer in the future,Mr.LU Zhuzhi’s prescription and medication rules of endometrial cancer were analyzed and summarized.LU Zhuzhi’s TCM prescriptions for the treatment of endometrial cancer in LU’s Clinical Experimental Records were collected and screened,and Excel software was used to establish the database.Association rules of IBM SPSS Modeler 18.0 were used to analyze commonly used drug combinations,and Cytoscape 3.8.0 and SPSS Statistics 22 were used for data mining analysis.Prescriptions involved 54 kinds of drugs,with a total use frequency of 515 times.The high frequency TCM medicine included Zhigancao(Radix Glycyrrhizae),Zhifupian(Radix Aconiti Lateralis Preparata),Sharen(Fructus Amomi),Baizhu(Rhizoma Atractylodis Macrocephalae),Danggui(Radix Angelicae Sinensis).The medicinal properties were mainly warm and hot,and the medicinal taste was mainly spicy and sweet.Drugs were used in large doses,with large span of drug dosage.The drugs mainly involved the spleen,liver,kidney.The core drug group was the Zhifupian-Baizhu-Sharen-Danggui-Weijiang(Rhizoma Zingiberis)-Zhigancao.In the cluster analysis,six cluster groups were obtained.The decision tree with the preparation of Zhifupian as the dependent variable showed the important position in the compatibility process of Cangzhu(Rhizoma Atractylodis),Puhuang(Pollen Typnae),Weijiang,Zhishengma(Rhizoma Cimicifugae).Using data mining technology can effectively analyze the prescription and medication rule of LU Zhuzhi in the treatment of endometrial cancer.Yang(阳)deficiency and internal cold are the root cause,and blood stasis and pain are the symptom.Therefore,in clinical treatment,attention should be paid to protecting and improving the middle-jiao(中焦),adjusting the rise and fall of Qi(气).Supporting Yang and suppressing Yin(阴)should be considered as the treatment ontology,and regulating Qi and dispersing blood stasis should be regarded as the methodology.
