摘　　要：偏头痛是神经内科常见的一种原发性头痛。其发病率高,起病缓慢,病情反复,缠绵难愈。患者发病时多为一侧疼痛,或左或右,疼痛时间可达4~72 h,同时会伴随有恶心、呕吐等症状,声、光刺激的情况下会加重头痛。西医认为此病的病因尚不明确,病机尚不十分清楚。王立平主任医师认为此病可分外感和内伤两类,外感头痛多以风邪为主;内伤头痛多责之于肝、脾、肾诸脏。本病主要由外感风邪、情志失调、饮食劳倦、久病致瘀的基础上造成的肝、脾、肾等脏腑功能紊乱导致。治疗上,强调辨病与辨证相结合,因于肝者,临床多见肝阳上亢;因于脾者,多见脾失健运,痰浊内阻,致清阳不升,浊阴不降;因于肾者,多因禀赋不足,或劳欲内伤,肾精亏虚,最终使脑失养,脉络失荣而成。用药多重视平抑肝阳、化痰祛瘀、补肾健脾和补益气血,从而使脏腑气血调和,其临床疗效甚佳。文章结合案例,介绍王主任治疗偏头痛的成功经验,以期为临床拓宽思路。Migraine is a common primary headache in neurology department.It has a high incidence rate.The onset is slow onset,and the condition is repeated,lingering and difficult to cure.Most patients suffer from pain on one side,that is,pain on the left or right side.The duration of pain can generally be as long as 4 hours to 72 hours,accompanied by nausea,vomiting and other symptoms.Sound and light stimulation will aggravate the headache.Western medicine believes that the etiology of this disease is not yet clear,and that the pathogenesis is not very clear.WANG Liping believes that this disease can be divided into two categories:exogenous infection and internal injury.The exogenous headache is mainly caused by wind evil,while the internal injuried headaches are mostly responsible for the liver,spleen and kidneys.This disease is mainly caused by the dysfunction of liver,spleen,kidney and other internal organs due to external wind evil,emotional disorders,improper diet,overstrain,and stasis caused by long-term illness.In treatment,the combination of disease differentiation and syndrome differentiation is emphasized.Due to the liver,Yang(阳)hyperactivity of the liver is often seen clinically.Because of the spleen,it is often seen that the spleen loses transport,and the phlegm is turbid and blocked internally,resulting in the stagnation of clear Yang and turbid Yin(阴).Because of the kidney,it is mostly due to insufficient endowment,or internal injury of exertion,kidney essence deficiency,and finally the brain is denourished.In terms of the medication,more attention is paid to calming liver Yang,resolving phlegm and blood stasis,tonifying kidney and spleen,and tonifying Qi(气)and blood,so as to reconcile Qi and blood of viscera.The clinical effect is very good.This article introduces Director WANG Liping’s successful experience in the treatment of migraine,with a view to broadening the clinical thinking.
