破格救心汤联合β-受体阻滞剂治疗气滞血瘀证冠心病心绞痛患者的效果及对抗氧化能力的影响

摘　　要：目的探究破格救心汤联合β-受体阻滞剂治疗冠心病心绞痛患者的效果及对抗氧化能力的影响。方法选取2019年3月至2021年3月浙江省临海市中医院医疗卫生服务共同体接诊的气滞血瘀证冠心病心绞痛患者168例,将其按照随机数字表法分为β-受体阻滞剂组、破格救心汤组和联合用药组,每组56例。治疗前及治疗2周后观察三组心肌损伤标志物脑钠肽(BNP)、心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶-MB(CK-MB)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、一氧化氮(NO)、超氧化物歧化酶(SOD)、丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)、脂质过氧化物(LPO)、中医证候评分及中医证候疗效。结果治疗后,三组BNP、cTnI、CK-MB水平低于治疗前,且联合用药组低于β-受体阻滞剂组、破格救心汤组(P<0.05)。治疗后,三组TNF-α、IL-6水平低于治疗前,且联合用药组低于β-受体阻滞剂组、破格救心汤组(P<0.05);三组NO水平高于治疗前,且联合用药组NO水平高于β-受体阻滞剂组、破格救心汤组(P<0.05)。治疗后,三组SOD、GSH-Px水平高于治疗前,且联合用药组高于β-受体阻滞剂组、破格救心汤组(P<0.05);三组MDA、LPO水平低于治疗前,且联合用药组低于β-受体阻滞剂组、破格救心汤组(P<0.05)。治疗后,三组心绞痛、肢体麻木、皮下瘀斑、情绪异常评分均低于治疗前,且联合用药组低于β-受体阻滞剂组、破格救心汤组(P<0.05)。联合用药组的中医证候疗效高于β-受体阻滞剂组、破格救心汤组(P<0.05)。结论气滞血瘀证冠心病心绞痛患者经破格救心汤与β-受体阻滞剂联合治疗后能够有效改善心肌损伤和机体炎症状态,增强抗氧化能力,在临床治疗中取得良好的效果。Objective To explore the effect of Poge Jiuxin Decoction combined withβ-blocker on the treatment of angina pectoris of coronary heart disease with qi stagnation and blood stasis syndrome and its anti-oxidation ability.Methods A total of 168 patients with angina pectoris of coronary heart disease with qi stagnation and blood stasis syndrome were selected from the Linhai Hospital of Traditional Chinese Medicine Medical and Health Service Community of Zhejiang Province from March 2019 to March 2021,and they were divided intoβ-blocker group,Poge Jiuxin Decoction group,and combined drug group according to random number table method,with 56 cases in each group.Brain natriuretic peptide(BNP),cardiac troponin I(cTnI),creatine kinase-MB(CK-MB),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),nitric oxide(NO),superoxide dismutase(SOD),malondialdehyde(MDA),glutathione peroxidase(GSH-Px),lipid hydroperoxide(LPO),traditional Chinese medicine syndrome score,and traditional Chinese medicine syndrome efficacy were observed in the three groups before treatment and two weeks after treatment.Results After treatment,the levels of BNP,cTnI,and CK-MB in the three groups were lower than before treatment,and those in the combined drug group were lower than those in theβ-blocker group and the Poge Jiuxin Decoction group(P<0.05).After treatment,the levels of TNF-αand IL-6 in the three groups were lower than before treatment,and those in the combined drug group were lower than those in theβ-blocker group and the Poge Jiuxin Decoction group(P<0.05);the level of NO in the three groups was higher than that before treatment,and that in the combined drug group was higher than that in theβ-blocker group and the Poge Jiuxin Decoction group(P<0.05).After treatment,the levels of SOD and GSH-Px in the three groups were higher than before treatment,and those in the combined drug group were higher than those in theβ-blocker group and the Poge Jiuxin Decoction group(P<0.05);the levels of MDA and LPO in the three groups were lower than before
