原发性腹膜后血管周细胞瘤1例报告

摘　　要：目的总结腹膜后原发性血管周细胞瘤诊治经验,提高诊疗水平。方法回顾贵州医科大学附属医院2022年3月28日收治的1例腹膜后肿物病人在住院期间的一般资料、病因、临床表现、辅助检查及诊疗结果,并进行综合分析。结果血管周细胞瘤临床表现无特异性,CT等影像学检查可以看出肿瘤的边界和范围以及周围有无组织侵犯及破坏,从而判断肿瘤的良恶性,但是无法确诊,最终结合术中探查情况及免疫组织化学结果确诊。结论发生于腹膜后的原发性血管周细胞瘤罕见,临床表现、影像学检查无特异性,术中探查及免疫组织化学结果是确诊该疾病的主要方式,治疗方案上仍然以手术为主,暂无明确指南及专家共识提出放化疗对血管周细胞瘤治疗的有效性。Objective To summarize the experience in the diagnosis and treatment of primary retroperitoneal hemangiopericytoma and to improve the level of diagnosis and treatment.Methods The general data,etiology,clinical manifestations,auxiliary examinations and diagnostic and treatment results of a patient with retroperitoneal masses admitted to the Affiliated Hospital of Guizhou Medical University on March 28,2022 were reviewed and comprehensively analyzed.Results The clinical manifestations of hemangiopericytoma are nonspecific.Imaging examinations such as CT can show the boundary and extent of the tumor as well as whether there is tissue invasion and destruction around it to determine the benign or malignant nature of the tumor;however,it is not possible to confirm the diagnosis,which is ultimately confirmed by combining intraoperative probing and immunohistochemistry results.Conclusions Primary hemangiopericytoma occurring in the retroperitoneum is very rare,and the clinical manifestations and imaging examinations are not specific.Intraoperative exploration and immunohistochemical results are the main modalities for confirming the diagnosis of this disease,and the treatment options are still based on surgery,with no clear guidelines or expert consensus suggesting the effectiveness of radiotherapy in the treatment of perivascular cell tumors.
