摘 要:目的探索瓦里安Rapidplan优化模块在胸上段食管癌容积旋转调强计划(VMAT)中的可行性及潜在优势。方法选取50例患者已执行放疗的胸上段食管癌VMAT计划,导入Rapidplan数据库,建立自动优化模型。另选10例胸上段食管癌VMAT计划对该模型进行独立验证。比较Rapidplan自动优化计划(RP)和传统人工计划(RG)的靶区和危及器官剂量学。结果Rapidplan可预测剂量分布,在靶区、心脏和肺受量相当的情况下,降低脊髓的受量。RP计划靶区的均质性指数(HI)差于RG计划,适形指数(CI)优于RG计划(P<0.05)。RP计划的脊髓外扩结构的最大剂量优于RG计划(P<0.05),总肺的V_(20)、V_(5)和RG计划差异无统计学意义(P>0.05)。RP计划优化效率优于RG计划(P<0.05),执行效率与RG计划差异无统计学意义(P>0.05)。结论Rapidplan自动优化模块能够很好地应用于胸上段食管癌VMAT计划。Objective To explore the feasibility and potential advantages of the Varian Rapidplan optimization module in the volumetric modulated art therapy(VMAT)for upper thoracic esophageal cancer.Methods Fifty patients with VMAT plan for upper thoracic esophageal cancer who had undergone radiotherapy were selected and imported into the Rapidplan database to establish an automatic optimization model.Another 10 cases of upper thoracic esophageal cancer VMAT plan were selected for independent validation of this model.The target area and organ endangerment dosimetry of Rapidplan automatic optimization plan(RP)and traditional manual plan(RG)were compared.Results Rapidplan can predict dose distribution and reduce spinal cord uptake when the target area,heart,and lung uptake are comparable.The homogeneity index(HI)of the RP plan target area was worse than that of the RG plan,and the conformity index(CI)was better than that of the RG plan(P<0.05).The maximum dose of the spinal cord expansion structure in the RP plan was better than that in the RG plan(P<0.05),and there was no statistically significant difference in the V_(20),V_(5),and RG plans of the total lung(P>0.05).The optimization efficiency of the RP plan is better than that of the RG plan(P<0.05),and there is no statistically difference in execution efficiency between the RP plan and the RG plan(P>0.05).Conclusion The Rapidplan automatic optimization module can be well applied to the VMAT plan for upper thoracic esophageal cancer.
